Unresolved Fever in a Patient With Diabetic Nephropathy

A patient with diabetic nephropathy stage 4, and chronic myeloid leukemia; were hospitalized due to fever, ascites, hepatomegaly and acute on chronic renal failure. Her chest X-ray revealed diffuse reticulonodular infiltration. Her erythrocyte sedimentation rate, C-reactive protein levels were found to be increased. The computed tomography resulted in peritoneal nodules and renal calcification. Peritoneal biopsy culture results revealed as Mycobacterium tuberculosis and with a 2-month anti-tuberculous treatment, the patient's renal function tests came to premorbid levels without dialysis. With this case report, a 50-year-old female diabetic patient with fever of unknown origin was discussed.

Diyabetik nefropati (evre 4), konik miyeloid lösemi tanılarıyla takip edilen hasta; ateş, asit, hepatomegali ve kronik zeminde akut böbrek yetmezliği gelişmesi üzerine hastaneye kabul edildi. Akciğer filminde difüz retikülonodüler infiltrasyon, eritrosit sedimentas-yon hızında ve C-reaktif protein düzeylerinde artış saptanan hastanın, abdomen bilgisayarlı tomografilerinde peritonda nodüller ve renal kalsifikasyon izlendi. Periton biyopsi kültürlerinde Mycobacterium tuberculosis üremesi saptanan hastanın, 2 aylık antitüberkü-loz tedavisi ile hemodiyaliz programına ihtiyacı kalmayıp, böbrek fonksiyon testleri premorbid düzeylere gelmiştir. Bu olgu ile sebebi bilinmeyen ateşle kliniğe başvuran 50 yaşındaki diyabetik bir kadın hasta tartışılmıştır.
