Elevated serum creatine phosphokinase levels in patients with hypotyhroidism. Overt hypoth-yroidism is known to be associated with increased serum creatine phosphokinase (CPK) levels. A strinkingly elevated CPK level was unusual. The aim of this study was to investigate the levels of CPK and eveluate the correlations betvveen CPK levels and age, Anti-TPO, thyro-id functions in patients with hypothyroidism Serum CPK levels were elevated in 50% (23/46) of the patients. In eleven of 23 (23.9%), serum CPK levels were över 2000IU/L, and a signifi-cant elevation in CPK levels (>5000 IU/L) was observed in four of them. Although ther was a positive correlation betvveen CPK and thyrotropin (TSH) levels, no significant correlation bet-ween CPK and thyroid hormones, Anti-TPO, and age. After L-thyroxine therapy, CPK levels gradually decreased to normal. It is concluded that overt hypothyroidism may be associated with highly elevated CPK levels, and hypothyroid myopathy

Hipotiroidi miyopatisinin yüksek serum kreatin fosfokinaz (CPK) değerleri ile birlikte olduğu bilinmektedir. Ancak dikkati çekecek yükseklikteki CPK değerleri nadirdir. Bu çalışmanın amacı 46 hipotiroidik hastada CPK düzeylerini gözden geçirmek ve bunların tiroid fonksiyonları ile ilişkisini değerlendirmektir. Hastaların %50'sinde (23/46) CPK değerleri yüksek bulunmuştur. Serum CPK değerleri bu hastaların 11 'inde 2000 IU/L üzerinde, ve bunların da dördünde aşırı yüksek (>5000IU/L) değerlerdedir. Yüksek serum CPK değerleri ile tirotropin (TSH) arasında pozitif bir ilişki bulunmasına rağmen, bu ilişki yaş, Anti TPO ve tiroid hormonları ile CPK arasında bulunamamıştır. L-Tiroksin tedavisinden sonra CPK düzeyleri de azalarak normale inmiştir. Sonuç olarak, aşikar hipotiroidi de CPK değerleri çok yüksek bulunabilir, bu nedenle mutlaka tiroid fonksiyon testleri ile hipotiroid miyopatisi diğer metabolik myopatilerden dışlanmalıdır.
