Combination syndrome occurs when the mandibular anterior teeth applie strenght to edentulous maksiller bone, patients with a completely edentulous maxilla is opposed by Kennedy Class I partially edentulous mandible. Combination syndrome –also called ‘Anterior Hipefonction Syndrome’ or ‘Kelly’s Syndrome’- characterized with including loss of bone from the anterior portion of the maxillary ridge, overgrowth of the tuberosities, papillary hyperplasia of the hard palatal mucosa, extrusion of mandibular anterior teeth, and loss of alveolar bone and ridge height beneath the mandibular removable partial denture bases. Combination syndrome have not be seen all the patiens with a completely edentulous maxilla is opposed by Kennedy Class I partially edentulous mandible. The mandibular removable partial denture decrease formation of combination syndrome with distributing the occlusal stres to hard and soft tissues otherwise patients disuse of mandibular removable partial denture aggravate the combination syndrome. The purpose of this clinical report is to present prothetic rehabilitation of the one patient with clinical and radyographic symptoms concerned about combination syndrome has applied to the clinic.

Kombinasyon sendromu üst çene tam dişsiz arkın karşısında alt çenede Kennedy Sınıf I diş eksikliğinin olduğu vakalarda, alt ön grup dişlerin dişsiz krete uyguladıkları kuvvetler sonucu meydana gelir. ‘Anterior hiperfonksiyon sendromu’ veya ‘Kelly Sendromu’ da denilen kombinasyon sendromu; üst çene ön bölgede kemik kaybı, tüberlerde aşırı büyüme, sert damak mukozasında papiller hiperplazi, alt ön dişlerde uzama ve alt çenede hareketli protez kulanımına bağlı olarak kret yüksekliğinde azalma ile karakterizedir. Kombinasyon sendromu üst çene tam dişsiz arkın karşısında alt çene Kennedy Sınıf I diş eksikliğine sahip hastaların hepsinde görülmez. Alt çeneye yapılan hareketli bölümlü protez okluzal stresleri yumuşak ve sert dokulara beraber dağıtarak kombinasyon sendromunun gelişmesini azaltırken, hastaların alt çene hareketli bölümlü protezlerini kullanmamaları kombinasyon sendromunu şiddetlendirmektedir. Bu çalışmada, kliniğimize başvuran kombinasyon sendromuna ait klinik ve radyografik bulguları taşıyan bir hastaya uygulanan protetik tedavi hakkında bilgi verilmiştir.
