ACUTE NECROTISING ULSERATIVE GINGIUITIS

Abstract (2. Language):
ANUG is the severe inflammatory periodontal disorder caused by plaque bacteria associated with diminished systemic
resistance. This type of gingivitis is limited to lesions involving gingival tissue with no loss of periodontal attachment and
bone loss. Three specific clinical signs must be present to diagnose ANUG : pain, necrosis of interdental papillae and
bleeding. Development of ANUG is closely associated with spesific predisposing factors ; immune suppression, acute psychological stress, smoking, malnutrition, pre-existing gingivitis, tissue trauma and AIDS favors the occurance of opportunistic infections. A 39 -year -old female patient with acme gingival lesions was referred to our clinics at the Department of Periodontology, Faculty of Dentistry, University of Istanbul. The patient used to smoke two packs of cigarettes daily. In the first medical visit she was documented thoroughly and ofnidazole (s: 2*2). etodolac (2*1). vitamin B (2*1). 3% H2Os (2*1). was prescribed to the patient. At the second and third visits supragingival scaling was performed under local anesthesia. Because of slow remission of the lesions another prescription was given including amoxicillin and clavulanate potassium (2*1). and chlorhexidme moulhrinse (2*1) at the fourth visit except of this the patient was referred to the biochemical laboratory for further analysis. At subsequent visits, with short periods, sub- and supragingival scaling was continued. The patient was taken to maintenance therapy and no other clinical signs of inflammation has been observed. At the final stage of treatment plan the patient was advised to continue her visits periodically. Special attention was given to inform the patient about oral hygiene procedures and adverse effects of smoking.

ANUG plak bakterilerinin yol açtığı ve azalan konak direnciyle ilişkili bir periodontal hastalıktır. Bu gingivitis tipinde
lezyonlar dişetiyle sınırlıdır, ataşman ve kemik kaybı yoktur. ANUG teşhisinde üç spesifik klinik belirti teşhis açısından
önem taşımaktadır. Bunlar, ağn, interdental papilde nekroz ve kanamadır. ANUG'un gelişimi bağışıklık sisteminin
baskılanması, akut psikolojik stres, sigara, beslenme bozukluğu, varolan bir gingivitis ve doku travması gibi belirli hazırlayıcı
faktörlerle yakından ilişkilidir. Ayrıca AİDS gibi immiin sistemi zayıflatan sendromlar ve yetersiz beslenme, bu tür
Iczyonların oluşumuna zemin hazırlamaktadır. Akut dişeti şikayetleri nedeni ile İstanbul Üniversitesi Dişhekimliği Fakültesi
Periodontoloji Anabilim Dalı kliniğimize başvuran 39 yaşındaki kadın hasta, anamnezindc günde 2 paket sigara içmekte
olduğunu bildirmiştir. İlk randevusunda hasta ayrıntılı şekilde dokümanıc edilmiş ve kendisine ornidazol (s: 2*2), etodolak
(2*1), B vitamini (2*1), %3'lük H2 0 2 (2*l)'den oluşan ilaç listesi reçete edilmiştir. İkinci ve üçüncü randevularda lokal
anestezi altında supragingival diş yüzeyi temizliği yapılmıştır. Hastalığın gerileme hızındaki yavaşlık nedeniyle dördüncü
randevuda daha geniş spektrumlu bir antibiyotik (amoksina+klavulanat potasyum) ve klorheksidin gargara (2*l)'daıı oluşan
ikinci bir reçete yazılmış ve hasla ileri tetkikler için biyokimya laboratuarına sevkedilmiştir. Sonraki randevularda kısa süreli
supra- ve subgingival diş yüzeyi temizliklerine devam edilmiştir. Hasta idame tedavisine alınmış olup, kontrollerde herhangi
bir iltihapsal değişim gözlenmemiştir. Tedavi planlamasının en son aşamasında hastaya kontrol randevularına devam etmesi
gerektiği belirtilmiştir. Hasta ağız hijyeni uygulamaları ve sigara kullanımının olumsuz etkileri konusunda ayrıntılı şekilde
bilgilendirilmiştir.
