To investigate the value of chemical shift MR imaging using qualitative analysis in the differentiation of adrenal adenomas from other adrenal masses. Forty-seven patients with 64 adrenal masses (adenoma: 44, nonadenoma: 20) were evaluated using chemical shift MR imaging. An experienced radiologist and a fellow nearing the end of training who were blinded to diagnosis and clinical data independently evaluated the images two times. The signal intensity of the lesion relative to the spleen was scored on a five-point scale. Cohen’s kappa values to determine the intra- and interobserver variation were estimated. The first observer classified the lesions correctly except one in both evaluations. The second observer misclassified two lesions in first evaluation and one in second evaluation. This method had excellent intra-observer and excellent inter-observer reproducibility. The safeness of the chemical shift MR imaging using qualitative analysis in differentiation of adrenal adenomas from nonadenomas is high.

Adrenal adenomların diğer adrenal kitlelerden ayırt edilmesinde, kalitatif değerlendirmenin kullanıldığı kimyasal şift MR görüntülemenin tanı değerini araştırmak. Kırk yedi olgudaki 64 adrenal kitlesi (44 adenom, 20 adenom-dışı), kimyasal şift MR yöntemiyle görüntülendi. Hastaların klinik bilgi ve tanılarından habersiz biri deneyimli, diğeri eğitim aşamasında olan iki radyolog, birbirlerinden bağımsız olarak iki farklı zamanda lezyonların intensitesini, referans organ olarak seçilen dalağın intensitesi ile görsel olarak karşılaştırdı ve 'beş nokta skalasına' göre puanlandırıldı. Gözlemciler arasındaki ve her bir gözlemcinin farklı zamanlarda yaptığı değerlendirmeler arasındaki değişkenliği belirlemek için, Cohen’nin kappa değerleri hesaplandı. Birinci gözlemci farklı zamanlarda yaptığı her iki değerlendirmede de, biri dışında tüm kitlelerin adenom-adenom dışı ayrımını yapabildi. İkinci gözlemci ise, ilk değerlendirmesinde 2, ikinci değerlendirmesinde 1 kitleyi yanlış karakterize etti. Yöntemin, gözlemciler arasındaki tekrar edilebilirlik değerleri ile gözlemcilerin farklı zamanlarda yaptıkları değerlendirmelere göre tekrar edilebilirlik değerleri mükemmeldi. Kimyasal şift MR görüntülemede kalitatif değerlendirme, adrenal adenomları adenom-dışı adrenal kitlelerden ayırmada güvenilir bir yöntemdir.
