The prime objective of an access cavity is to provide an unobstructed or straight-line pathway to the apical foramen. Accidents such as excess removal of tooth structure or perforation occur during attempts to locate canals or as a direct result of failing to achieve straight-line access to the canals, l^ack of attention to the degree of axial inclination of a tooth in relation to adjacent teeth and to alveolar bone may result in either gouging or perforation ol the crown or the root at various levels. Failure to hold the bur parallel to the long axis of a tooth also may cause gouging or perforation. This case report prescnis restoration and two year follow up mandibular second premolar and first molar perforated by the dentist.

Diş seri dokularının fazla miktarda u/aklnştınlması veya perforasyon gibi ka.y.alar ka-lULİkırı lokal ize ermek için teşebbüsler şifasında veya kanallara dü/. bir «iriş elde etmek için hasırı sizliğin bir sonucu olarakta uluşnr. Komşu dikler ve alveolar kemikle ilişkideki bir dişin aksini eğiminin derecesine ilgi eksikliği kron veya kökün değişik seviyelerinde ya madde kaybı ya da perforas-vonla sonuçlanabilir. Bir dişin uzun aksına frezio paralel lu-tulma.smdaki hata madde kaybına veya perforasyona neden olabilir. Bu vakada diş hekimi tarafından perforc edilen mandibular ikinci premolar ve birinci molamı restorasyonu ve iki yıllık takibi sunulmaktadır.
