APPLICATION OF SOFT DENTURE LINING MATERIAL TO A GERIATRIC PATIENT HAVING EXTREMELY RESORBED RESIDUAL RIDGE

Resilient-lined complete dentures are a viable option, especially for patients with severely reduced residual alveolar ridges. These patients cannot tolerate conventional hard acrylic resin denture bases due to poor conditions like a thin and non-resilient mucosa. The aim of this case report is to present our experience on application of soft denture lining material that increases clinical success in a similar case. 68 years old female patient having a problem of occlusion, function and stability in her prosthesis applied to our clinic with a complaint of not being able to use her lower prosthesis. After the evaluation of the present denture, it was observed that in addition to the poor occlusion, the patient also had flat residual ridge in the anterior section and negative residual ridge in the posterior sections of her lower jaw caused by extreme resorption. At the radiographic examination of the patient it was determined that the canine was impacted and because of extreme resorption the extraction of it was ineligible and there was an apparent bone ridge. Conventional complete prosthesis for the upper jaw and resilient-lined complete prosthesis for the lower jaw were performed. During the clinical controls after the treatment, it was observed that her chewing function and aesthetic expectations were satisfied. Soft lining material appling to the lower complete dentures lead to good clinical results.

Yumuşak astar maddesi ile astarlanan tam protezler, özellikle tam protez için iyi bir destek sağlayamayan rezorbe alveoler kretlere sahip hastalar için iyi bir seçenektir. Bu hastalar, ince ve esnek olmayan mukozaları nedeniyle geleneksel sert akrilik rezin protez kaidelerini tolere edemezler. Bu vaka raporunun amacı, benzer bir olguda, yumuşak astar maddesi uygulamasının klinik başarıyı artırdığı yönündeki deneyimimizi sunmaktır. Protezinde okluzyon, fonksiyon ve stabilite problemi olan 68 yaşındaki kadın hasta kliniğimize alt tam protezini kullanamama şikayetiyle başvurmuştur. Mevcut protezi değerlendirildiğinde hatalı okluzyonun yanı sıra, hastanın özellikle alt çenesinde aşırı rezorbsiyona bağlı olarak anterior bölgede düz, posterior bölgede negatif kretler gözlenmiştir. Radyografik muayenesinde alt sağ kaninin gömülü olduğu, aşırı rezorbsiyon nedeniyle ekstraksiyonunun uygun olmadığı ve bu bölgede belirgin kemik çıkıntısı olduğu belirlenmiştir. Üst çeneye geleneksel tam protez, alt çeneye daimi yumuşak astar maddesi uygulanmış tam protez yapılmıştır. Tedavi sonrası yapılan klinik kontrollerde hastanın çiğneme fonksiyonu ve estetik beklentilerinin sağlandığı gözlenmiştir. Alt tam protezlere yumuşak astar maddesi uygulaması iyi klinik sonuçlar ortaya koymaktadır.
