The prognosis of stroke has been claimed to be worsened at the presence of electrocardiographic findings. An association with mortality and ECG finding has not been well documented in patients with stroke. This case control study covers 87 consecutive patients with ischemic stroke and 87 control patients without stroke seen in any inpatient clinic other than cardiology between October 1995 June 1996. Results.- There was no difference between the patient and control groups in terms of sex and age. In the case of ischemic stroke, ECG changes were identified in 62.1 %, whereas the rate was 29.9% in the control group. The difference was statistically significant (Odds ratio, 3.84; 95% confidence interval, 1.95 to 7.60, p<0.0001). The mostly seen ECG finding was myocardial ischemia. Case fatality rate in all patients after 6 months of follow up was 29.9%. The fatality rates of the patients with and without ECC changes (38.9% and 15.2% respectively) were significantly different (p<0.05). Conclusion.- Cardiac consequences of stroke are likely to contribute to the stroke mortality. Patients with acute stroke should receive continuous cardiac monitoring until the cardiac problem and ECG findings have resolved.

Akut stroklu hastalarda sekonder EKG değişikliklerinin görüldüğü ve bu bulguların prognoz ile ilişkili olduğu bildirilmektedir. İleriye dönük ve vaka kontrollü olarak planlanan bu çalışmada, Ekim 1995-Haziran 1996 tarihleri arasında Nöroloji'de yatırılarak izlenen ve ilk kez strok geçiren 87 serebral infarktlı (Sİ) hastanın EKG bulguları, aynı dönemde Cerrahpaşa Tıp Fakültesi'nin Kardiyoloji dışında herhangi bir kliniğine yatan, aynı yaş grubunda 87 kişilik kontrol hastasının EKG bulguları ile karşılaştırılmıştır. Hastaların %52.9 u erkek, % 47.1 i kadındır. Yaşları 31-91 arasındadır (ortalama yaş: 65.5±11.9). Kontrol grubunun % 49.4 ü erkek, % 50.6 sı kadındır. Yaşları 31-87 arasındadır (ortalama yaş: 64.5±9.1). Sİ'li hastaların % 62.1 inde EKG bozukluğu saptanırken bu oran kontrol grubunda % 29.9 bulunmuştur. Aradaki fark istatistiksel olarak anlamlıdır (p<0.0001). EKG de en sık miyokardial iskemi ile ilgili bulgular görülmüştür. Saptanan EKG değişikliklerinin hepsinin stroka sekonder olduğunu belirlemek mümkün olmamakla birlikte, kontrol grubundan anlamlı ölçüde farklı oranlar saptanması aradaki ilişkinin belirgin olduğunu göstermektedir. EKG değişikliği saptanan hastalarda ilk altı ay içinde fatalite oranı % 38.9, EKG si normal olanlarda % 15.2 dir. Aradaki fark belirgindir (p<0.05). Bu bulgular nöroloğun kardiyolojik bulgulara gereken hassasiyeti göstermesinin prognozu önemli ölçüde etkileyeceğini göstermektedir.
