Stones are well known complication of urinary diversion, with the majority of calculi being composed of struvite or carbonate apatite. Continent diversions seem particularly at risk. Diversion stones are often an incidental finding on imaging studies. However, when symptomatic, patients may complain of hematuria, pyuria, pressure sensation and difficult catheterization.
Multiple options are available for the treatment of diversion calculi, including stone basketing, percutaneous cystolitotomy, open cystolitotomy and extracorporeal shock wave lithotripsy (ESWL). Here in we report a case of a bladder stone in a 51 year old man with continent diversion due to difficult catheterization after augmentation cystoplasty. Radiological examinations revealed a bladder stone with a diameter of 3x2 cm. Percutaneous cystolitotripsy was performed under general anesthesia. The patient was stone free and discharged from the hospital at the postoperative third day without any complication. We recommend the percutaneous cystolitotripsy as a safe and easy way with high stone free rates in patients with continent diversion.

Taşlar üriner diversiyonların iyi bilinen komplikasyonlarından olup, genellikle karbo-nat yada struvit yapısındadırlar. Kontinan di-versiyonlar ise özellikle risk altında bulunmak-tadırlar. Diversiyon taşları genellikle görüntü-leme yöntemlerinde rastlantısal olarak saptanır. Ancak hastalar semptomatik olduklarında he-matüri, pyüri, zor kateterizasyon ve basınç his-sinden yakınabilirler.
51 yaşında erkek hastaya, 17 yıl önce og-mentasyon ileosistoplasti sonrası zor kateteri-zasyon nedeniyle Mitrafonoff kateterize edile-bilen stoma ile kontinan diversiyon oluşturul-muş. Hastanın hematüri ve zor kateterizasyon yakınmaları üzerine çekilen direkt üriner sis-tem grafisinde ogmente mesanede 3x2 cm bo-yutlarında kalkül saptandı ve hastaya perkütan sistolitotripsi gerçekleştirildi.
Tekniğine uygun olarak perkütan sistolito-tripsi gerçekleştirilen hastanın postop 2. gün sistostomi kateteri alınarak 3. gün servisimiz-den taşsız olarak taburcu edildi. Postoperatif dönemde herhangi bir komplikasyon görülme-di.
Diversiyon taşlarında, perkütan sistolito-tripsinin güvenli ve kolay uygulanabilen, has-tane kalış süresini kısaltan ve yüksek taştan arınma oranı sağlayan bir teknik olarak kulanı-mını önermekteyiz.
