Comparison of gabapentine and gabapentine-tramadol combination in acute herpetic neuralgia

Abstract (2. Language):
Objectives: Acute herpes zoster associated pain is an important risk factor for postherpetic neuralgia (PHN). Anticonvulsants, antidepressants
and opioids are shown to be effective in prevention of PHN. But, the effects of these drugs in acute herpetic pain and in the prevention
of PHN are not known. In this study, the efficacy of gabapentine and gabapentine tramadol combination was compared in acute herpetic
pain and in the prevention of PHN.
Methods: 39 patients with acute herpes zoster pain (during 4th week of the disease) taken antiviral treatments were randomized to two
groups. In group G, gabapentine was started as 300-900 mg/day gradually titrated in three days and continued on maximum 3600 mg/day
in case. In group GT, tramadol together with gabapentine (started as same in group G) was started as 50-150 mg/day and titrated to maximum
400 mg/day in case and stopped when VAS<3. The assessment of burning, electrical and stabbing pain were done by VAS (visual analog
scale; 0=no pain, 10= most imagenable pain) and the presence of allodynia was assessed with light touch. The assessment times were
before the treatment and 1., 2., 3., and 4. Weeks and 6. Months after the treatments. On 6. months, VAS≥3 was accepted as PHN.Results: In comparison of burning, electrical and stabbing pain between groups, mean VAS and the presence of allodynia were statistically
significantly low in group GT (p<0.05). No statistically significant difference was present between groups in mean VAS on 6 months
(p>0.05). The gabapentine consumption was higher in group G than group GT (p<0.05). The incidence of side effects was high in group GT
especially within 2 weeks.
Conclusion: Gabapentine tramadol combination was more effective than gabapentine alone in acute herpetic neuralgia but, both treatments
prevented PHN in a similar manner.

Amaç: Herpes zostere bağlı akut ağrının, postherpetik nevraljide (PHN) bir risk faktör olduğu düşünülmektedir. Antikonvülzanların, antidepresanların ve opioidlerin postherpetik nevraljide etkin olduğu bildirilmiştir. Ancak, bu ilaçların akut herpes zosterde etkinliği ve PHN riskini
azaltıp azaltmayacağı bilinmemektedir. Bu çalışmada gabapentin ile gabapentin tramadol kombinasyonunun herpes zoster enfeksiyonunun
akut döneminde ağrı kontrolünündeki etkinliği ve postherpetik nevralji riski sınandı.
Yöntem: AHZ ağrısı nedeni ile hastalığın ilk 4 haftasında antiviral tedavi görmüş olarak başvuran 39 hasta iki gruba ayrıldı. Grup G hastalarına gabapentin 300-900 mg/gün üç gün içinde titre edilerek başlandı ve gereğinde en fazla 3600 mg/gün olacak şekilde tedaviye devam
edildi. Grup GT’ye gabapentin ile eş zamanlı olarak tramadol 50-150 mg/gün dozlarda titre edilerek verildi ve gereğinde en fazla 400
mg/gün olacak şekilde devam edildi. Grup GT’de VASı3 olduğunda tramadol uygulaması sonlandırıldı ve tedaviye en son aldığı dozda gabapentin
ile devam edildi. Yanıcı, çakıcı, batıcı ağrı VAS (vizüel analog skala: 0= ağrı yok, 10= dayanılmaz ağrı) ile, allodini varlığı ise “hafif
dokunma” ile değerlendirildi. Değerlendirmeler tedavi başlamadan önce, 1., 2., 3.ve 4. haftaların sonunda ve altıncı ayda yapıldı. Altıncı
aydaki değerlendirmede VAS≥3 ise PHN kabül edildi.
Bulgular: Gruplar karşılaştırıldığında, 1., 2., 3., 4. haftalardaki yanıcı, çakıcı ve batıcı ağrı VAS ortalamaları ve allodini varlığı grup GT’de
grup G’ye göre istatistiksel olarak anlamlı derecede düşüktü (p<0.05). Altıncı ayda ise ortalama VAS değerleri gruplar arasında herhangi bir
istatistiksel fark göstermemekteydi (p>0.05). Her iki grubun gabapentin tüketimi tüm değerlendirme dönemlerinde grup G’de daha yüksekti
(p<0.05). Grup GT’de yan etki görülme oranı ilk iki haftada grup G’ye göre istatistiksel olarak anlamlı yüksekti (p<0.05).
Sonuç: Gabapentin tramadol kombinasyonu tek başına gabapentine göre akut herpetik nevraljide daha etkilidir ancak her iki tedavi de PHN
riskini aynı derecede azaltmaktadır.
