In this study, results of the anti-incontinence surgery performed due to stress type urinary incontinence were evaluated. Of all the patients performed antiincontinence surgery due to genuine stress incontinence in different time periods between 1991 and 2004, 196 patients who can be contacted on phone were evaluated with the same questionnaire form. Eighty one patients performed Burch colposuspension between 1991 and 1998, 40 patients performed pubovaginal sling between 1995 and 2004, and 75 patients performed tension free vaginal tape between 2001 and 2004 were reached. In the questionnaire form 10, 5 and 4 questions were about continence, probable complications and patient satisfaction, respectively. Success rate was calculated considering only patients with no signs or symptoms of incontinence after surgery. In the Burch colposuspension group 28 (36%) of the 81 patients followed up with a mean period of 53.6 (38-94) months declared incontinence. In the pubovaginal sling group 12 (30%) of the 40 patients followed up with a mean period of 42.6 (7.1-107.9) months declared incontinence. In the tension free vaginal tape group 8 (11%) of the 75 patients followed up with a mean period of 21.6 (6-38.9) months declared incontinence. The most effective method in the surgical repair of urodynamical stres incontinence seems to be pubovaginal sling in the long term. Long term results of tension free vaginal tape should be seen to compare with the other treatment modalities.

Bu çalışmada stres ile idrar kaçırma nedeniyle antiinkontinan cerrahi girişim uygulanan hastaların sonuçları değerlendirildi. Stres ile idrar kaçırma şikayeti nedeniyle 1991 ile 2004 yılları arasında değişik zamanlarda antiinkontinan cerrahi girişim uygulanan hastalardan telefon ile temas sağlanabilen 196 hastaya aynı sorgulama formu doldurtularak elde edilen sonuçlar değerlendirildi. Burch kolposüspansiyonunun 1991 ile 1998 yılları arasında uygulandığı hastaların 81'ine, pubovajinal sling'in 1995 ile 2004 yılları arasında uygulandığı hastaların 40'ına ve "tension free vaginal tape"in 2001 ile 2004 yılları arasında uygulandığı hastaların 75'ine ulaşıldı. Sorgulama formunda 10 soru kontinans durumu ile ilgili, 5 soru gelişebilecek komplikasyonlarla ilgili, 4 soru hasta memnuniyeti ile ilgiliydi. Başarı oranı, sorgulamada cerrahi sonrası hiçbir şekilde idrar kaçırmadığını ifade eden hastalar dikkate alınarak hesaplandı. Burch kolposüspansiyon uygulanan grupta ortalama takip süresi 53.6 (38-94) ay olan 81 hastadan 28'i (%36) idrar kaçırması olduğunu belirtti. Pubovajinal sling uygulanan grupta ortalama takip süresi 42.6 (7.1-107.9) ay olan 40 hastadan 12'si (%30) idrar kaçırması olduğunu belirtti. "Tension free vaginal tape" uygulanan grupta ortalama takip süresi 21.6 ay (6- 38.9) olan 75 hastadan 8'i (%11) idrar kaçırması olduğunu belirtti. Ürodinamik stres inkontinans cerrahi tedavisinde uzun dönemde etkinliği en yüksek olan girişim pubovajinal sling gibi görünmektedir. "Tension free vaginal tape" sonuçlarını diğer tedavi yöntemleri ile karşılaştırmak için geç dönem sonuçlarını beklemek gerekmektedir.
