Lacrimal drainage system can be obstructed by many reasons. One of the reasons of this obstruction is lacrimal sac tumor. Dacryocystorhinostomy was performed to our patient because of chronic dacryocystitis. During surgery it was noticed that lacrimal sac mucosa was thicker than its usual thickness, and biopsy was taken. Pathologic evaluation of the specimen revealed extranodal marginal zone Bcell lymphoma (MALT lymphoma). There was no evidence of systemic lymphoma, and the patient was treated with 42 Gy irradiation. This case demonstrated that chronic dacryocystitis may be the first sign of lymphoma. This rare condition should be taken into consideration in evaluating patients with chronic dacryocystitis.

Göz yaşı akım sistemi birçok nedenle kesintiye uğrayabilir. Bu nedenlerden birisi de lakrimal kese tümörüdür. Olgumuza kronik dakriyosistit nedeniyle dakriyosistorinostomi ameliyatı uygulandı. Ameliyat sırasında kese mukozasının normalden kalın olduğu görüldü ve biyopsi alındı. Patolojik inceleme sonucu olgu ekstranodal marginal zon B hücreli lenfoma (MALT lenfoma) tanısı aldı. Sistemik lenfoma bulgusuna rastlanmayan hasta 42 Gy radyoterapi uygulanarak tedavi edildi. Bu olgu lenfomanın ilk belirtisinin kronik dakriyosistit olabileceğini göstermiştir. Kronik dakriyosistitli hastaları değerlendirirken bu durum dikkate alınmalıdır.
