The central giant cell granuloma constitutes approximately 7% of the benign jaw tumors. The clinical behavior of central giant cell granuloma ranges from a slowly growing asymptomatic lesion to an aggressive lesion manifesting with pain, local bone destruction, root resorption, or displacement of teeth. Recurrence risk is high after insufficient treatment. In this case report a 28-year-old male patient with central giant cell granuloma in whom free osteocutaneous fibula flap was performed is presented. Local curettage and marginal resection may not provide a complete treatment in cases of central giant cell granuloma with aggresive course and resistant to radiotherapy. Instead, we suggest restoration with free osteocutaneous flap following an extended resection and long term clinical and radiological follow up.

Benign çene tümörlerinin yaklaşık %7’sini santral dev hücreli granülomlar oluşturur. Klinik olarak yavaş seyirli, asemptomatik lezyonlardan, agresif formlarında ağrı, lokal kemik destrüksiyonları, diş kökü rezorpsiyonu ve diş kaybına neden olabilecek kadar geniş bir yelpazede gözlenebilir. Yetersiz tedavi sonrası nüks oranı yüksektir. Bu yazıda serbest osteokütan fibula flebi onarım uygulanan 28 yaşında erkek olgu sunulmuştur. Radyoterapiye cevap vermeyen, agresif seyreden santral dev hücreli granülom vakalarında küretaj ve marjinal rezeksiyon tam kür sağlayamayabilir. Bunun yerine geniş rezeksiyon ve uzun süreli klinik ve radyolojik takip sonrası serbest osteokütan fibula flebi ile onarımın uygun olacağını düşünmekteyiz.
