Visual Evoked Potentials in Vitamin B12 Deficiency: Results of a Preliminary Study

Abstract (2. Language):
Objective: Aim of this study was to investigate the possible relationship between serum
vitamin B12 levels and P100 latency and amplitudes with visual evoked potentials (VEP) in
patients with B12 vitamin deficiency without optic neuritis (ON).
Material and Methods: Patients who presented to the outpatient neurology clinic of
Çanakkale Onsekiz Mart University Research Hospital underwent initial evaluation. Among
those, complete blood count, full biochemistry (blood glucose, liver and kidney function
tests, electrolites) and thyroid function tests were performed to the patients with serum B12
vitamin levels <200 pg/dl and without optic neuritis who accepted to participate.
Electrophysiological evaluations included VEP. Data were organized in SPSS 15.0
programme and statistical analysis were performed in this programme. Non-parametric
correlation test and Spierman’s rho tests were used for statistical analysis.
Results: Patients enrolled the study consisted of 13 (68.4%) men and 6 (31.6%) women.
Mean age was 52.68±15.10 years (range=19-75 years). Mean vitamin B12 levels
were160.41±44.59 pg/ml (range=78-230 pg/ml). There was no statistically significant
correlation between serum vitamin B12 levels and P100 latency and amplitudes (p>0.05).
Conclusion: Our results reveals that there is no correlation between the VEP values and the
severity of vitamin B12 deficiency in the absence of ON in patients with vitamin B12
deficiency. Study still continues, but results of the preliminary study are shared in order to
discuss the value of VEP in patients with vitamin B12 deficiency without ON.

Amaç: Bu çalışmada, serum B12 vitamini eksikliği olan ve muayene ile optik sinir tutulumu
saptanmamış olan hastalarda, görsel uyarılmış potansiyel (GUP) çalışması yapılarak, B12
vitamin düzeyleri ile GUP çalışmasında elde edilen P100 latans ve amplitüd değerleri
arasında olası bir ilişkinin varlığını saptamak amaçlandı.
Gereç ve Yöntemler: Çanakkale Onsekiz Mart Üniversitesi Araştırma ve Uygulama
Hastanesi Nöroloji Polikliniğine başvuran hastalar ön değerlendirmeye alındı. Çalışmaya
katılmayı kabul eden, serum B12 vitamini düzeyi <200 pg/dl olan ve optik nöropatisi
saptanmayanlar hastalara tam kan sayımı, geniş biyokimya paneli (kan şekeri, karaciğer ve
böbrek fonksiyon testleri, elektrolitler) ve tiroid fonksiyon testleri yapıldı. Elektrofizyolojik
incelemeye alınan hastalara GUP çalışmaları uygulandı. Elde edilen veriler SPSS 15.0
programına aktarılarak istatistiksel analizler bu programda yapıldı. İstatistiksel metot olarak
non-parametrik korelasyon testi ve Spierman’s rho testi kullanıldı.
Bulgular: Çalışmaya katılan hastaların 13’ü erkek (%68.4), 6’sı kadındı (%31.6). Yaş
ortalaması 52.68±15.10 (sınırlar=19-75 yaş) olarak bulundu. Ortalama vitamin B12
düzeyleri 160.41±44.59 pg/ml (sınırlar=78-230 pg/ml) idi. Serum vitamin B12 düzeyleri ile
P100 latans ve amplitüdleri arasında istatistiksel olarak anlamlı korelasyon saptanmadı
(p>0.05).
Sonuç: Çalışmamızın sonuçları, ON yokluğunda B12 vitamini eksikliği olan hastalardan elde
edilen GUP değerleri ile B12 vitamin eksikliği derecesinin korelasyon göstermediğini telkin
etmektedir. Çalışmamız devam etmektedir fakat ön çalışma sonuçlarımız, ON olmaksızın
B12 vitamini eksikliği olan hastalarda GUP’ in yerini tartışmaya açmak açısından
paylaşılmaya değer bulunmuştur.
