The main indications for surgery in thyroid disease include malignancy, cytologically suspicious diagnosis, tracheo esophageal compression and cosmetic reasons. 90 patients who underwent to surgery for nodular goiter were studied prospectively. Preoperatively, 76 patients were evaluated by thyroid USG, 30 patients by thyroid scintigraphy and all the patients by fine needle aspiration biopsy (FNAB). Thyroidectomy indications in our cases were malignancy or suspicious cytologic results in 26, clinical findings of malignancy in 4 patients, hypertyroid in 16, compression symptoms in 22, recurrent goiter in 7, and cosmetic reasons in 15 patients. Cytohistopathologic correlation was established in these cases. In the malign and suspicious FNAB results the sensitivity and the specificity of the procedure was determined as 90% and 79%, respectively. In the evaluation of postoperatively diagnosed 10 malign patients; 9 of the 48 solitary nodules (18.75%), 9 of the 14 fixed nodules (64.29%) and all of the 3 patients having lymphadenopathy were confirmed as malignancy. The malignancy rate in man (36%) was significantly higher than the women (8.70%) (p<0.05). Malignancy risk was higher in solid nodules evaluated by USG and warm, cold nodules by scintigraphy (p< 0.05). In conclusion, FNAB can be used effectively in the evaluation of thyroid nodules and by correlating with the other malignancy risk factors. Thyroidectomy can be performed on the suspicious and malignant cytologic results so operation rate in the benign thyroid nodules can be decreased. Tiroid nodüllerinde; malignensi ve malignite şüphesi, hipertiroidi, hava yollarına, özöfagusa bası bulgularının olması ve kozmetik nedenlerle tiroidektomi uygulanabilir. Prospektif olarak yapılan bu çalışmada 90 nodüler guatrlı hastaya tiroidektomi uygulandı. Preoperatif dönemde olguların 76'sına tiroid USG, 30'una tiroid sintigrafisi ve tümüne ince iğne aspirasyon biopsisi (İİAB) uygulandı. Olguların 26'sı İİAB sonuçlarına göre, 4'ü klinik bulgular ile malignite düşünülerek, 16'sı hipertiroidi, 22'si bası bulguları, 7'si nüks guatr ve 15'i de kozmetik nedenlerle opere edildi. Sitolojik sonuçlar ile histolojik sonuçlar karşılaştırıldı. Malignite şüpheli ve malign olgular ele alındığında İİAB'ın sensitivitesi %90, spesifitesi %79 bulundu. Postoperatif malignite saptanan 10 olgunun değerlendirilmesinde; muayenelerinde soliter nodül bulunan 48 olgudan 9'unda (%18.75), 14 fikse nodülden 9'unda (%64.29) ve lenfadenopatisi olan 3 olgunun hepsinde (%100) malignite bulundu. Erkeklerde malignite görülme oranı (%36) kadınlara göre (%8.70) anlamlı yüksek bulundu (p< 0.05). Tiroid USG ile solid olarak belirlenen nodüller ve sintigr afi ile ılık ve soğuk olarak belirlenen nodüller de malignite oranı daha yüksek bulundu (p<0.05). Sonuç olarak nodüler tiroidli olguların İİAB ile tetkikinin diğer bulgularla birlikte değerlendirildiğinde benign olgulara da yapılmakta olan tiroidektomilerin önemli ölçüde azalacağı tesbit edilmiştir.
