Dysplasia and neuroendocrine differentiation in gallbladder carcinomas

In this study, 8 gallbladder carcinomas among 435 cholecystectomy specimens which were examined in the Pathology Department of İnönü University Medical School, between 1992 and 1996, were reviewed. In microscopic examination, three mucinous carcinomas, three well and one moderately differentiated adenocarcinomas and one adenosquamous cell carcinoma were seen. Dysplasia was detected in two cases adjacent to tumor tissues. Five cases had stage V, 1 had stage IV and, 2 had stage III tumors. Tumor cells were focally positive for Chromogranin-A in 2 adenocarcinomas and 2 mucinous carcinomas. Although it is reported that neuroendocrine differentiation has no effect on prognosis of gallbladder carcinoma, the number of Chromogranin-A positive cases was more common in advanced stage tumors in our study. Bu çalışmada 1992-1996yılları arasında, İnönü Üniversitesi Tıp Fakültesi Patoloji Anabilim Dalı'nda 435 safra kesesi materyalinde saptadığımız 8 safra kesesi karsinomu gözden geçirildi. Mikroskobik incelemede 3 müsinöz karsinom, 3 iyi ve 1 orta diferansiye adenokarsinom ile 1 adenoskuamöz karsinom görüldü. İki olguda tümör komşuluğunda displazi saptandı. Olguların 5 'i evre V, 1 'i evre IV ve 2 'si evre III idi. iki adenokarsinom ve 2 müsinöz karsinom olgusunda tümör hücrelerinde Kromogranin-A ile fokal pozitiflik vardı. Nöroendokrin diferansiyasyonun safra kesesi karsinomlarında prognoza etkili olmadığı bildirilmesine rağmen çalışmamızda ileri evredeki tümörler arasında Kromogranin-A ile pozitif olgu sayısı daha fazla bulundu.
