Oral candidial flora and effectiveness of prophylactic treatment in pediatric oncologic patients

Candida species are the most common causes of fungal infections in immunocompromised patients. The purpose of this study was to investigate oral candidial flora and effectiveness of the prophylactic treatment in pediatric oncologic patients. Thirty four patients with haemotologic or solid malignancy enrolled in the study. The samples were taken from patient's oral mucosa and incubated in Sabouraud's agar. Isolated yeast colonies were typed with classical and disc diffusion methods. Fourteen patients had positive culture for candidiasis. The distribution of candidal species with the classical method was 11 (78,6%) C. albicans, two (14,3%) C. kefyr and one (7,1%) C. (Torulopsus) glabrata. The disc diffusion method identified 12 (85,7%) C. albicans and two (14,3%) C. glabrata from the specimens. The ratio of yeast colony production were 33,3% in patients with prophylactic treatment and 44% in patients without prophylaxis. As a result, the follow-up of oncologic patients both with mucosal candidal culture and systemic fluconasole administration as a prophylaxis would be suitable.

İmmün sistemi baskılanmış kişilerde fırsatçı mantar enfeksiyonlarının en sık nedeni kandidalardır. Bu çalışmada, pediatrik onkolojik hastalarda oral kandida florası ve profilaktik tedavinin etkinliğinin incelenmesi amaçlanmıştır. Bu amaçla çalışmaya hematolojik veya solid tümörü bulunan 34 hasta alındı. Olguların oral mukozasından alınan sürüntüler kloramfenikollü Sabouraud besiyerine ekildi. Kandida tiplendirilmesi klasik ve disk diffüzyon yöntemi ile yapıldı. Olguların 14'ünde maya üremesi görüldü. Klasik yöntemle yapılan tiplendirme sonununda bunların 11 (%78.6)'inin Candida albicans, ikisinin (%14.3) C. kefyr, birinin (%7.1) C. (Torulopsus) glabrata olduğu, disk diffüzyon yöntemi ile ise, 12 (85.7)'sinin C. albicans ve ikisinin (%14.3) C. glabrata olduğu görüldü. Profilaktik tedavi alanların %33.3'ünde maya hücresi ürerken profilaktik tedavi almayanlarda bu oran % 44 idi. SSonuç olarak, onkolojik olguların oral mukozasından kandida için kültür alınarak tak p ed lmes n n ve prof lakt k olarak s stem k flukonazol verilmesin in uygun olacağını düşünmekteyiz.
