Aim of this study was to define the difference between the effectivity of two proton pump inhibitors plus dual antibiotics therapy for eradication of Hp in patients with peptic ulcer. For this study, 44 subjets with documented Hp infection of the stomach were recruited. Patients were treated as follows: 1) omeprazol 20 mg bid plus amoxicillin 1gr bid plus clarithromycin 500 mg bid for one week (OAC)> 2) lansoprazole 30 mg bid plus amoxicillin 1 gr bid plus clarithromycin 500 mg bid for one week (LAC). After 4 weeks upper gastro ntest nal endoscopy and b opsy was repeated for h stolog cal and endoscop c evelaut on. Hp erad cat on rates were respect vely % 86.9 and % 90.4 n OAC and LAC groups. Both PPI-based tr ple therapies tested in this study were safe and effective in curing Hp infection. There were not any stat st cally s gn f cant d fferences between two groups. OAC and LAC reg mens resulted n h gh cure rates and thus may be cons dered the treatment of cho ce.

Gelişmekte olan ülkelerde önemli bir sağlık sorunu olan Helicobacter pylori (Hp) infeksiyonu ülkemizde de bölgeden bölgeye farklılık göstermekle birlikte, yüksek oranlarda izlenmektedir. Biz bu çalışmamızda iki farklı proton pompa inhibitörünün Hp eradikasyonu ve ülser iyileşmesindeki ektinliğini kıyaslamayı amaçladık. Kliniğimize dispeptik yakınmalarla başvuran ve yapılan endoskopilerinde gastrik veya duodenal ülser saptanan 44 hasta rastgele iki guruba ayrıldı. İki gurup arasında yaş ve cinsiyet yönünden farklılık yoktu. Birinci gruptaki 23 hastaya omeprazol 2x20 mg + amoksisilin 2x1 gr + klaritromisin 2x 500 mg (OAK), ikinci guruptaki 21 hastaya ise lansoprazol 2x30 mg + amoksisilin 2x1 gr + klaritromisin 2x500 mg (LAK) birer hafta süre ile verildi. Tedaviden önce ve tedavi bitiminden 4 hafta sonra endoskopi yapılarak biyopsi alındı. Hp (-) hastalarda eradikasyon sağlandığı kabul edildi. OAK verilen gurupta Hp eradikasyonu % 86.9 (20/23), LAK gurubunda ise % 90.4 (19/21) d ve k gurup arasında stat st ksel olarak anlamlı fark yoktu. Sonuç olarak klaritromisin, amoksisilin ve proton pompa inhibitörlerinin bir hafta süre ile verilmesi ile yüksek oranlarda Hp eradikasyonu sağlanmaktadır (%86.9-%90.4). Kullanılan proton pompa inhibitörlerinin farklı olması sonucu etkilememektedir. İlaç seçiminde maliyetin göz önünde bulundurulmasının daha uygun olacağı sonucuna varıldı.
