Acute Rupture ofthe Spleen during Orthotopic Liver Transplantation for Fulminant Hepatitis B. We report on a patient who had spontaneous splenic rupture during total vascular exclusion (TVE) in orthotopic liver transplantation for fulminant hepatitis B. To the best of our informa-tion, this complication has been reported in a single liver transplant case previously . The most probable reason is acute elevation of portal pressure. The spleen should be observed carefully for a few minutes after the initiation of TVE. Also, splenic rupture should be considered im-mediately in an unexpected hemorrhage at any time in the course of TVE.

Fulminan B hepatiti sebebiyle yapılan ortotopik karaciğer nakli(OKN) sırasında, total vasküler eksklüzyon(TVE) uygulandıktan sonra, spontan dalak rüptürü gelişmiştir. Bu komplikasyon, OKN sırasında, daha önce bir defa bildirilmiştir. En olası sebep, portal basıncın akut olarak yükselmesidir. TVE'nin başlangıcından sonra dalak birkaç dakika süreyle gözlenmelidir. TVE boyunca herhangi bir zaman noktasında beklenmedik bir kanama olduğunda, akla dalak rüptürü gelmelidir
