Decreased polymorphonuclear leukocyte deformability in tlıe elderly. Infectious diseases occur more often and are usually more severe in the elderly than in younger adults, probably because aging affects host defenses. Inflammation is characterized by increased local blood flow, increased permeability of blood vessels and rapid mobilisation of polimorphonuclear neutrophil leukocytes. In order to migıate to the site of inflammation, the PNLs pass fhrough the vessel walls. During this process, they must alter their morphology, a.property called "deformability". In this study we investigated PNL deformability in the elderly. The study group involved 21 patients (65-88 years old) and the control group 20 healthy individuals (18-62 years old). After centrifugation of whole blood on Ficoll gradient, the celi pellet (RBCs and PNLs) was subjected to osmotic lysis of the RBCs. After centrifugation were PNLs resuspended in Ringer solution at a concentration of 1x109/1. Celi suspensions were pumped with a constant flow rate of 0.5 ml/min at room temperature (20-23°C) through polycarbonate filters with anominal pore size of 5 um. Prior to the filtration of a celi suspension, buffer solution alone was filtered to obtain the filtration pressure for suspending medium (Po.) The slope of the filtration pressure- time curve between 20 and 60 s after the start of pump was used as a parameter of WBC rigidity. In the study group, PNL rigidity was significantly higher than controls (0.22+0.03 vs. 0.14+0.05, p<0.0001). Also, we detected a moderate positive correlation betvveen age and PNL rigidity. There was a weak negative correlation between PNL rigidity and albumin and trygliceride levels. We conclude that PNL rigidity increases with aging, which can result in impaired PNL extravasation and poor response to infection in the elderly.

Yaşlılarda infeksiyonlar gençlerden daha sıktır ve genellikle daha ciddi seyreder, çünkü muhtemelen yaşlandıkça savunma mekanizmaları bozulmaktadır. İnflamasyon sürecinde lokal kan akımı ve damarların geçirgenliği artar, polimorf nüveli nötrofiller (PNL) hızla damar inflamasyon yerine göçerler. PNL'lerin damarı terk ederek, damar duvarından inflamasyon bölgesine göçebilmesi için deforme olabilmeleri gereklidir. Bu çalışmada yaşlı PNL'lerinin deforme olabilme yetenekleri araştırılmıştır. Çalışma grubu yaşları 65-88 arasında değişen 21 hastadan, kontrol grubu 18-62 yaşlan arasında 20 sağlıklı erişkinden oluşmaktaydı. Olgulardan alman venöz tam kan örneği Ficoll ile santrifüj edildikten sonra, elde edilen hücre karışımı içindeki eritrositler osmotik lizise uğratıldı. PNL'lerin Ringer laktat içindeki 1x109/1 konsantrasyondaki çözeltisi, oda sıcaklığında (20-23°C), 0.5 ml/dak sabit hızda, 5 pm'lik gözenekli polikarbonat filtrelerden geçirildi. Daha önce, ortamın filtrasyon basıncını belirlemek için (Po), tampon sıvısı filtre edildi. Lökosit rijiditesini gösteren parametre olarak, 20- 60. saniyeler arasındaki filtrasyon basıncı-zaman eğrisinin eğimi kullanıldı. Yaşlı grubunda PNL'lerin rijiditesi kontrol grubuna göre artmıştı (sırasıyla, 0.22+0.03, 0.14+0.05, p<0.0001). PNL rijiditesi ile yaş ile arasında anlamlı, orta derecede, pozitif korelasyon, PNL rijiditesi ile albümin ve trigliserid düzeyleri arasında anlamlı, zayıf, negatif korelasyon bulundu. Sonuçlar yaşla birlikte PNL'lerin rijiditesinin arttığını göstermektedir, bu yaşlılarda PNL'lerin ekstravaze olma yeteneğinde azalmaya yol açarak infeksiyonlarla başa çıkmadaki yetersizliğe katkıda bulunuyor olabilir.
