Adenoid cystic carcinomas (ACCs) are rare malignant salivary gland tumors. They can arise in a variety of sites in oral cavity including salivary glands but more than 50% of cases originate from minor salivary glands and especially seen in hard palate. ACC generally occur in middle-aged individuals, especially fifth through seven decades of life, and both gender are affected. Clinical symptoms, radiological examinations, surgical operation and radiotherapy treatment of the maxillary sinus infrastructural adenoid cystic carcinoma in 73 years old female patient who applied to University of Istanbul, Faculty of Dentistry, Department of Oral Medicine and Surgery because of painfull, ulcerative lesion in the left side of maxilla that was discouraging the usage of her total prosthesis, was presented and the importance of dentists' awareness of the long-lasting lesions in oral cavity was emphasized in this report.

Adenoid kistik karsiııom, nadir görülen malign bir tükürük bezi tümörüdür. Ağız içerisinde tükürük bezlerinin bulunduğu her bölgede oluşabilir ancak vakaların %50'den fazlası minor tükürük bezlerinde gelişir ve en çok sert damakta görülür. Genellikle orta yaşın üstündeki bireylerde, yaşamın 5-7. dekadlannda ortaya çıkar ve her iki cinsi de etkiler. Vaka raporumuzda, sol üst çenesinde ortaya çıkan ve üst total protezini kullanmasını engelleyen ağrılı, ülseratif lezyon nedeniyle İstanbul Üniversitesi Diş Hekimliği Fakültesi Ağız, Diş, Çene Hastalıkları Bilim Dalı kliniğine başvuran 73 yaşındaki kadın hastada tespit edilen maksilter sinüs infrastrüktUrel adenoid kistik karsinom olgusunun klinik bulguları, radyolojik tetkikleri, cerrahi operasyonu ve bunu takiben uygulanan radyoterapi tedavisi sunulmuş ve diş hekimierinin ağız içinde oluşan ve uzun süre iyileşmeyen lezyonlar konusunda daha dikkatli olmaları gerektiği vurgulanmıştır.
