COMPARISON OF FOUR CORNERED FLAPS PREPARED BY TWO DIFFERENT INCISIONS IN ENDODONTIC SURGERY

The aim of this study was to compare the effects of four-cornered (laps prepared by two different incisions, which are frequently used in endodontic surgery, on periodontal health of the related teeth and patient comfort. Thirty two. systemically and periodontally healthy patients were included in this study. All of the patients had a periapical lesion in one maxillary tooth with a diameter of 0.5-1 cm radiologically. Following the root-canal treatment performed preoperatively on the day of surgery, the following parameters were measured to compare with the postoperative relevant measurements: thickness and width of attached gingiva, attachment loss and depth of periodontal pocket. Patients were divided into 2 groups: sulcular incision was used for group I while submarginal scalloped incision was used for group 2 for the preparation of a four cornered Hap. Immediately postoperatively and on the 7'1' postoperative day. visual analogue scale (VAS) (between 0 and 10) scorings of pain, speaking and chewing abilities were recorded. The gathered data was statistically analyzed using paired samples t lest. There was no statistically significant difference in VAS scores between the two groups. Group 1 had significantly higher attachment loss and gingival recession measurements when compared with group 2. There is no difference in four cornered flaps prepared either by sulcular or submarginal scalloped incisions when patient comfort following endodontic surgery is taken into consideration, however: submarginal scalloped incision may be advantageous especially in esthetic regions since it does not yield to attachment loss and gingival regression.

Bu çalışmanın amacı endodontik cerrahide sıklıkla kullanılmakla olan iki farklı insizyon ile hazırlanan dörtgen llehin hasta konforuna ve ilgili dişlerin periodontal sağlıklarına yönelik etkilerini karşılaştırmaktır. Sistemik ve periodontal açıdan sağlıklı, üst çene ön dişlerinin sadece bir tanesinde radyolojik olarak 0.5-1 cm çapında kök ucu lezyonu olan 32 hasta çalışmaya dahil edildi. İşlem günü preoperatif kanal tedavisini lakihen operasyona alınan hastaların ilgili diş bölgesindeki bukkal dişeti kalınlığı ve genişliği, dişeti çekilmesi, periodontal cep derinliği posiopcratif değerlerle karşılaştırmak üzere ölçüldü. Hastalar uygulanacak insizyona göre 2 gruba ayrıldı: birinci gruba siliktiler insizyonla. ikinci gruba ise submarjinal insizyonla dörtgen llcp hazırlandı. İşlemden hemen sonra ve 7. postoperatif günde hastalara ağrı. konuşabilire ve çiğneycbilme durumları ile ilgili 0-10 arasında derecelendirilen görsel ölçeklendirme değerlendirmesi (VAS) yapıldı, filde edilen veriler bağımlı örnekler için t testi ile değerlendirildi. İki grup arasında görsel ölçeklendirme değerlendirmesi bulgularına göre istatistiksel olarak fark bulunamadı Birinci grupta 2. gruba göre işlemden 5 halta sonra istatistiksel olarak anlamlı derecede ataşman kaybı ve dişeti çekilmesi gerçekleştiği görüldü. Hndodonlik cerrahi amaçlı yapılan girişimlerde postoperatif hasta konforu açısından sulkuler veya submarjinal insizyonla hazırlanan dörtgen Heplerin birbirlerine üstünlüklerinin olmadığı ancak ataşman kaybı ve dişeti çekilmesine neden olmaması nedeniyle özellikle estetik bölgede yapılacak işlemlerde submarjinal insizvonun daha avantajlı olabileceği görüldü.
