Ball Attachment Screw Fracture Case Due to False Restoration Planning

Screw fracture is one of the mechanical complications that occur in implant restorations. This is a situation that generally arises after screw loosening depending on metal fatigue. There are plenty of factors that lead to screw loosening. As for this case, a series of complications that come along due to an inaccurate implant placement on the opposing jaw causes screw fracture of a ball attachment. After the fractured screw specimen was extracted with endodontic files, an ultrasonic cavitron and a probe for common examination without detriment to the screw hole, a new ball attachment was applied to the implant. Because of financial insufficiencies and an ongoing cervical discal hernia treatment, patient didn't want to have additional implants in her maxilla and new prostheses. Also she didn't want to replace ball attachments with locator attachments which used in cases that show limited vertical height. For this reasons existed prostheses was made available for usage then delivered to the patient. Screw loosening is a more recognized complication then screw fracture. A loose screw that is not retightened would be fractured. Adequate treatment planning, well arranged of abutment screw loosening and fracture. occlusal contacts and screw tightening with correct torque value will minimize the incidence.

Vida kırığı implant restorasyonlarında görülen mekanik komplikasyonlardan biridir. Çoğunlukla vida gevşemesini takiben metal yorgunluğuna bağlı olarak ortaya çıkan bir durumdur. Vida gevşemesine neden olabilecek pek çok faktör bulunmaktadır. Bu vakada ise karşıt çenede hatalı şekilde konumlanmış bir implantın neden olduğu bir dizi komplikasyon sonucunda ball ataşman vidası kırılmıştır. Kırık vida parçası kanal eğeleri, ultrasonik kavitron ve bir muayene sondu yardımıyla vida yuvasına zarar vermeden çıkarılmış, yeni bir ball ataşman implanta bağlanmıştır. Hasta maddi durumunun yetersiz olması ve boyun fıtığı tedavisi görüyor olması sebebiyle üst çeneye ilave implantlar yapılmasını, ball ataşmanların - dikey boyutun sınırlı olduğu vakalarda kullanılan - locator tutucularla değiştirilmesini ve yeni protezlerin yapımını reddetmiştir. Bu sebeplerle mevcut protezler kullanılabilir hale getirilip hastaya teslim edilmiştir. Vida gevşemesi, vida kırığından daha sık meydana gelen bir komplikasyondur. Gevşeyen vida sıkılmadığı takdirde kırılabilir. Uygun tedavi planlaması, iyi düzenlenmiş oklüzal temaslar ve vidanın doğru tork kuvvetiyle sıkılması, vida gevşemesi ve kırığının görülme sıklığını azaltacaktır.
