Introduction and Purpose:We have analyzed the reporting process of nutritional assessment procedures. We used FMEA
technique in order to evaluate these procedures and we were able to reduce the risk potential to 45%. The purpose was to analyze
opportunities to decrease potential errors in nutritional assessment processes. The aim of this study was to understand the logic of
FMEA technique and reduce the risk during radiological reporting process and to take safety measures.
Method: FMEA is a systemic approach to determine the dragging or failing parts of a process and the necessary precautions to be
taken to make those processes safer. The aim is to define the possible errors of a process and to redesign the process and take
safety measures to prevent these errors.
Findings and Result: A risk priority number is calculated for each defined error. Pareto analysis is used to find out the severity of
the procedures. The prioritized error types in this study were; Primary physician does not ordered the dietician consultation and
dietician is not being included in the process of treatment. For each defined error a cause and effect diagram was used. An
improvement plan and solutions for the main purposes were developed.
The risk priority number related to each error type is recalculated by using FMEA technique. The total risk priority number has
decreased to 9743 from 17794 and the potential risk has been reduced to 45%. Instructions and revisions have been realized and
all employees of the nutrition assessment process have been trained.

Giriş ve Amaç:Yatan hastaların “nutrisyon (klinik beslenme) değerlendirme süreci”ndeki potansiyel hata türleri FMEA (Hata Türleri
Etkileri Analizi) tekniği kullanılarak analiz edilmiş, gerçekleştirilen iyileştirme faaliyetleri sonucu potansiyel riskler % 45 oranında
azaltılarak süreç iyileştirilmiştir.
Bu çalışmada amaç yüksek riskli süreçlerde proaktif risk azaltma yaklaşımları oluşturmak, FMEA tekniğini anlamak, çalışma
grubunun bu analitik sürecin adımlarını anlamasını sağlamak, Nutrisyon değerlendirme sürecindeki risk azaltmaya yönelik fırsatları
belirlemek ve bu sürecin “yüksek riskli” olarak belirlenen adımlarına ilişkin önlemler almaktır.
Gereç ve Yöntem: FMEA, bir sürecin daha güvenli çalışması için ne yapılması gerektiğini belirlemeye yönelik sistematik bir
yaklaşımdır. Bu yaklaşımda, süreçlerdeki olası hataları değerlendirmek ve süreçlerin tasarımı ve / veya yeniden tasarımında bir
hata ortaya çıkmadan önce hataları önlemek ana hedeftir. Yapılan bu çalışmada, bu teknik yöntem olarak kullanılmıştır.
Bulgular ve Sonuç: Süreçte oluşabilecek hata türleri belirlenmiş, hatanın hasta üzerinde oluşturacağı olası zararlar tanımlanmış
ve her hata türüne ait risk öncelik puanı hesaplanmıştır. Pareto analizi ile belirlenen hata türlerinde önceliklendirme yapılmıştır.
Önceliklendirilen hata türleri; Primer doktorun tedavi ihtiyacı duymaması ve destek tedaviye başlamaması, Primer Doktorun
"Konsültasyon Raporu" üzerinde diyetisyen konsültasyon isteğini belirtmemesi, Diyetisyenin tedavi sürecine dahil edilmemesi
olarak belirlenmiştir. Önceliklendirilmiş her bir hata türü için, hatanın “ana neden(ler)”i neden-sonuç diyagramları kullanılarak
belirlenmiştir. Ana nedenlere yönelik çözüm önerileri belirlenerek uygulamaya alınmıştır.
Her hata türüne ait risk öncelik puanı yapılan iyileştirmeler sonucu yeniden hesaplanmıştır. FMEA tekniği kullanılarak süreç riski
17794’den 9743’e puana düşürülmüş, potansiyel risk % 45 oranında azaltılmıştır. Talimatlar ve formların revizyonları
gerçekleştirilerek, süreçte yer alan çalışanlar, iş akışındaki değişiklikler doğrultusunda eğitilmiştir.
