A rare case of hemothorax: Due to thoracic Evving's sarcoma

An 11 - year old boy presented to the emergency room with severe dyspnea and left upper chest pain. The initial chest radiograph shovved a massive left pleural effusion, opacities in the left hemithorax and lytic areas İn the left 3rö rib. A thoracocentesis demonstrated a hemothorax. Despite treafment with blood tansfusions and aspiration of hemothorax, the patient's clinical status vvorsened and developed dyspnea. A thoracic computed tomograghic scan demonstrated a mass in the İntrathoracic extrapulmonary space and a lytic lesion İn the 3rd rib. A posterolateral thoracotomy was performed. Base on histologic findings, evving's sarcoma was diagnosed. He was asymptomatic throughout the 11 months follovv - up. İntrathoracic extrapulmonary thoracic Evving's sarcoma vvith spontaneous hemothroax is extremelyare.

Bir aydan beri sol üst göğüs ağrısı olan 11 yaşında erkek çocuğu şiddetli solunum sıkıntısıyla acil servise başvurdu. Çekilen akciğer radyografisi sol masif plevral efüzyonu,sol hemitoraks opasitesi ve sol 3. kotta litik alan¬ları gösterdi. Torasentezle hemotoraks demonstre edildi. Hastanın klinik durumunda kan replasmanına ve hemo¬toraks aspire edilmesine rağmen rahatlama olmadı. Bilgisayarlı toraks tomografik inceleme (BT) intratorasik ekstrapulmoner yerleşimli kitle ve 3. kotta litik lezyonun olduğu izlenimini verdi. Hastaya acil şartlarda posterolate-ral torakotomi uygulandı. Kitle göğüs duvarı ile çıkarıldı. Histolojik incelemesinde geniş alanlarda yuvarlak, hiperkromatik, nisbeten küçük hücrelerin vasküler bir stroma içerisinde yerleştikleri İzlendi. Bu bulgularla Ewing sarkomu tanısı kondu. Olgu cerrahiden sonra, 11 aylık takibinde asemptomatiktir. Masif hemotoraksla birlikte olan İntratorasik ekstrapulmoner Ewing sarkomu, oldukça enderdir.
