This study evafuated the potentiai of Tc-99m sestamibi vvhole body scan as an alternative to vvhole body I-131 scan for well differentiated thyroid cancer patients prior to 1-131 ablation and post abiation states. We evaiuated 115 patients vvith differentiated thyroid cancer who had total or near total thyroidectomy. Ali patients had undergone Tc-99m sestamibi whole body scanning. high resolution neck uttrasonography prior to conventional 1-131 vvhole body scanning, thyrogiobulin, and antithyroglobulin values measured. Also ali patients had neck and ehest CT vvıthout contrast agent The results shovved that Tc-99m sestamibi vvhole body images are highly sensitive and specific in dif¬ferentiated thyroid carcinoma patients vvithout thyroid supression ff4-offj, compared to conventional 1-31 vvhole body seans (kappa=0.68 p<0.00 Ij. Neck USG vvith Tc-99m sestamibi İs comparable vvith conventional iodine based vvhole body studies also compared to high dose post -ablation 6th day I-131 vvhole body seans. Neck USG inereases sen-sitivity and decreases specifîcity of Tc-99m sestamibi seans. Clinical investigations had to be made to evaluate the effı-cieney of Tc-99m sestamibi in patients receiving thyroid supression.

Bu çalışma ablasyon öncesi ve sonrasında iyi differansiye tiroid kanserli hastalarda alternatif tarama yöntemi olarak Tc-99m sestamib/'nin kullanılıp kullanılamayacağını değerlendirmiştir. Bu nedenle total veya totale yakın tiroidektomı uygulanan 115 differansiye tiroid kanseri hastası çalışmaya alindi- Tüm hastalara Tc-99m sestamibi tüm vücut tarama, yüksek rezoiüsyoniu boyun USG. serum tiroglobulin ve antitiroglobuiin ölçümleri ve kontrastsız boyun ve toraks tomo¬grafileri yapıldı. Sonuçlar göstermiştir ki Tc-99m Sestamibi tüm vücut taramalar, 1-131 taramalarla karşılaştırıldığında tiroid supresyonu almayan (T4off) hastalarda yüksek derecede sensitif ve spesifiktir jkappa -0.68 ÇKO.001). Boyun USG ile birlikte Tc-99m sestamibi yapılması, yüksek doz 1-131 ablasyonu sonrası 6. gün taramalarla karşılaştırılabilir etkinliğe sahiptir. Boyun USG, Tc-99m Sestamibi'nin sensitivitesini arttırırken spesifısitesini azaltır. Ancak Tc-99m Sestamibinin tiroid supresyonu alan hastalarda etkinliğinin değerlendirilmesi için klinik çalışmalara ihtiyaç vardır.
