Surgical treatment of pyopneumothorax developed due to rheumatoid arthritis

Aim: We report the management of one patient with rheumatoid arthritis developed pyopneumothorax. Case Report: Fifty seven-year old male patient with rheumatoid arthritis for six years was admitted to our emergency unit with complaints of fever, dyspnea on exercise, coughing, continuing for last 10 days. Right hydropneu¬mothorax was observed in chest radiography. Thoracentesis was performed initially. The patient was treated by chest tube drainage. The thoracotomy was planned because of the lungs were not expansed by the performed treatment. The thicked pleura was removed at the operation. Conclusion: Pathological findings demonstrated rheumatoid nodules.

Amaç: Piyopnömotoraks gelişen romatoid artritli bir olgunun sunumunu yapmak. Olgu Sunumu: Altı yıldır roma¬toid artrit hastalığı olan 57 yaşında erkek hasta acil servise 10 gündür devam eden öksürük, ateş, nefes darlığı şikayeti ile başvurdu. Hastanın çekilen radyografisinde sağda hidropnömotoraks olduğu tespit edildi. Hastaya torasentez yapıldı ve kapalı su altı drenajı uygulandı. Uygulanan tedavi ile akciğerler ekspanse olmayınca torako-tomi kararı verildi. Operasyonda kalınlaşan pleura çıkarıldı. Sonuç: Patolojik tetkikte romatoid nodüller görüldü.
