Is it necessary to take a posteroanterior chest X-ray before central vein catheterization for hemodialysis in patients with chronic renal failure?

Aim: Temporary hemodialysis catheters inserted into the internal juguler vein are commonly used in patients who need emergent hemodialysis. During this procedure, although it is very rare, the patient may have anatomical differences. Therefore, before the procedure, it should be studied whether the patient has any cardiovascular abnormality or not. Case report: Dextrocardia was detected in control chest X-ray of a 55 year-old patient, who was in routine hemodialysis programme and had a history of 10 arteriovenous fistula opera¬tions and multiple transient and permanent catheter insertions because of chronic renal failure. Conclusion: For this reason, we suggest posteroanterior chest x-ray as the routine technique since it is the simplest diagnostic method.

Amaç: Acil hemodiyaliz uygulanması gereken hastalarda yaygın olarak internal juguler venden konulan geçici hemodiyaliz kateterleri kullanılmaktadır. Bu girişim yapılırken nadir de olsa hastanın anatomik durumu farklılık arz edebilmektedir. Bu nedenle girişim öncesi hastada bir kalp damar anomalisi olup olmadığı araştırılmalıdır. Olgu sunumu: 55 yaşında, kronik böbrek yetmezliği nedeni ile rutin hemodiyaliz programında olan ve daha önce 10 kez arteriovenöz fistül girişimi ve çok sayıda geçici ve kalıcı hemodiyaliz kateteri girişimi uygulanan hastanın kontrol posteroanterior (PA) akciğer grafisinde dekstrokardi saptandı. Sonuç: Biz bu amaçla en basit tanı yönte¬mi olan PA akciğer grafisinin rutin olarak kullanılmasını önermekteyiz.
