Acute fatty liver of pregnancy is a rare disease that may lead to serious maternal and fetal complications. It is mostly seen in the third trimester and rarely seen in the postpartum period. The etiopathogenesis is not known. Multifactorial and genetic factors are thought to be responsible. The differential diagnosis include: preeclampsia, cholestatic jaundice and viral hepatitis. Treatment is supportive care following delivery. We presented a case of 21 years old, primigravida patient in her 34th week of pregnancy who had elevated liver enzymes, all other laboratory tests were normal on admission. Laboratory tests of the patient deteriorated within short period and acute fatty liver of pregnancy developed.

Gebeliğin akut yağlı karaciğer hastalığı nadir görülen, gebeliğin en sık üçüncü trimesterinde ve nadiren postpartum dönemde kendini gösteren, ağır maternal ve fetal komplikasyonlara yol açan bir hastalıktır. Etyopatogenezi halen bilinmemektedir. Multifaktoriyel ve genetik nedenlerden söz edilmektedir. Tanısı preeklampsi, kolestatik sarılık ve viral hepatitlerden ayrıcı tanı alması ile mümkündür. Tedavisi ise doğumu takiben destek tedavisidir. Biz makalemizde 21 yaşında, 34 haftalık ilk gebeliği olan, kliniğimize geldiğinde karaciğer enzim yüksekliği dışında anormal laboratuar bulgusu olmayıp takiplerinde kısa sürede laboratuar testleri ileri derece bozulan ve gebeliğin akut yağlı karaciğer hastalığı tanısı alan bir olguyu sunduk.
