DETERMINATION OF FRACTURE RESISTANCE OF FIXED RESTORATIONS PREPARED WITH FIBER-REINFORCED COMPOSITE AND CERAMIC

Introduction: Fiber-reinforced composite resins and lithium-disilicate glass ceramics may be used to restore the lost tissues of teeth. Inlay-retained fixed partial prostheses are conservative restorations because of less preparation of teeth and protection of periodontal health. Objective: To compare the fracture resistance of resin-bonded inlay-retained fixed partial prostheses prepared with resin- impregnated fiber-reinforced composite, bar fiber-reinforced composite and lithium-disilicate glass ceramic. Material and Method: A light-polymerized fiber-reinforced composite (^lite/Bisco/USA) with resin-impregnated fiber (Everstick CB/StickTech/Finland) (Groupl) and bar fiber (Tescera Rod/Bisco/USA) (Group2), and IPS Empress-II glass-ceramic (Ivoclar-Vivadent/Liechtenstein) (Group3) (n=7) were used in the study. Class II cavities were prepared on 2nd premolar (occlusodistal) and 2nd molar (occlusomesial). Teeth were placed with 10mm space. They were duplicated with sensitive impression material (Optosil-Xantopren/Heraeus-Kulzer/Germany). Mold was prepared with Ni-Cr alloy. Inlay-retained fixed partial prostheses were prepared on cast dies. Specimens were tested with Universal test machine (Lloyd-LRX-Universal/Fareham/England, 1.0mm/min, 250 kgf load cell) . One-Way-ANOVA and Duncan Multiple Comparison Test were used for statistical analyses (p<0,05). Results: Difference was not found between Group2 (359,504±43,940N) and Group3 (304,286±100,667N, p>0.05). The fracture resistance values of Groupl (463,914±82,738N) were statistically higher than Group2 (359,504±43,940N) and Group3 (304,286±100,667N) (p<0.05). Cohesive fractures were commonly observed. Conclusion: All restorations with light-polymerized composite reinforced with resin-impregnated fiber and bar fiber, and lithium dislicate-glass-ceramic were durable to chewing forces. It was thougth that, they can be safely used to restore the loss of teeth.

Giriş: Fiberle güçlendirilmiş kompozit rezinler ve lityum disilikatla güçlendirilmiş cam seramik materyaller, madde kaybına uğramış dişlerin restorasyonlarında sıklıkla kullanılmaktadırlar. İnley destekli sabit parsiyel restorasyonlar, dişlerde preparasyon miktarının az tutulabildiği ve dişlerin ve periodontal sağlığın daha fazla korunabildiği konservatif restorasyonlardır. Amaç: Rezin emdirilmiş fiber ve bar şeklinde fiberle güçlendirilmiş kompozit ve lityum disilikatla güçlendirilmiş cam seramik materyallerle hazırlanan ve rezinle yapıştırılan inley destekli sabit parsiyel restorasyonların basma kırılma dirençlerinin karşılaştırılmasıdır. Gereç ve Yöntem: Çalışmada; rezin emdirilmiş fiber (Everstick CB/StickTech/Finland) (Grup1) ve bar şeklinde fiberle (Tescera Rod/Bisco/USA) güçlendirilmiş ışıkla sertleşen kompozit (iElite/Bisco/USA) (Grup2) ve IPS Empress-II cam seramik (Ivoclar-Vivadent/Liechtenstein) (Grup3), (Grup1,2,3/n=7) kullanıldı. 2.daimi küçük azıya oklüzodistal ve 2.daimi büyük azıya oklüzomezyal Sınıf II kaviteler açıldı. Dişler, 10mm arayla yerleştirilerek hassas ölçü maddesiyle (Optosil-Xantopren/Heraeus-Kulzer/Germany) ölçü alındı, Ni-Cr alaşımdan metal kalıp hazırlandı, alçı daylar üzerinde inley destekli sabit parsiyel restorasyonlar hazırlandı, basma kırılma dirençleri Universal test cihazıyla (Lloyd-LRX Universal/Fareham/England)(1.0mm/dk, 250 kgf yükleme kapasitesi) test edildi. Veriler, Tek-yönlü ANOVA ve Duncan-Çoklu Karşılaştırma Testleriyle istatistiksel olarak değerlendirildi (p<0,05). Bulgular: Grup2 (359,504±43,940N) ve Grup3 (304,286±100,667N) grupları arasında istatistiksel farklılık gözlenmedi (p>0.05). Grupl'den sağlanan kırılma direnci değerleri (463,914±82,738N), diğer gruplardan sağlanan değerlerden (359,504±43,940N), (304,286±100,667N) istatistiksel olarak yüksek bulundu (p<0.05). Örneklerde genel olarak koheziv kırık tipine rastlandı. Sonuç: Çalışmada 2.daimi küçük azı ve 2.daimi büyük azı dişlerinde hazırlanan Sınıf II kavitelerde, rezin emdirilmiş fiber veya bar şeklinde fiberle güçlendirilmiş kompozit rezin ve lityum disilikat cam seramikle hazırlanan inley destekli sabit parsiyel restorasyonların tümünün çiğneme kuvvetlerine dayanıklı oldukları ve kayıp daimi dişlerin/dokularının restore edilmesi amacıyla kullanılabilecekleri fikrine varıldı.
