Intraosseous Infusion Using the Bone Injection Gun in a Natural Mass Disaster

The aim of this prospective non-randomized study is to evaluate the adequacy and success rate of general circulation access through intraosseous lines in adults and children, in emergency approaches using an automatic device, the bone injection gun (BIG). Adult and child cases were prospectively selected after Düzce-Bolu earthquake who arrived in emergency service of GATA and required emergency or semi-emergency vascular access, in whom intravenous peripheral cannulation could not be established within a reasonable period of time. Twenty one victims, aged one to 46 years, underwent the procedure. They all had multiple injuries and two of them underwent emergency cardiopulmonary resuscitation. In all cases the needle was inserted into the area of the tibial tuberosity except one case which the needle was inserted at the distal end of the radial bone. The success rate for an adequate insertion was 100 %. No pain or complications from the procedure were observed in this series. In conclusion, the use of the BIG, which is an impact, high speed automatic needle insertion device, provides a higher success rate of systemic circulation access via the intraosseal route in the victims of the mass disaster, and the impact penetration of a sharp and small trocar needle reduces the pain sensation of the bone to a minimum level in semi-conscious patients.

Bu randomize olmayan prospektif çalışmanın amacı otomatik bir alet olan kemik enjeksiyon tabancası ile acil yaklaşımlarda, întraosseöz yol üzerinden genel dolaşıma girişin, yetişkinlerde ve çocuklarda uygunluğunu ve başarı düzeyini değerlendirmektir. Olgular, Düzce-Bolu depreminden sonra GATA acil servisine getirilen ve makul bir zaman içinde intravenöz kanülasyonun sağlanamadığı, acil veya yarı acil damar yolu gerektiren yetişkin ve çocuk yaralılar arasından prospektif olarak seçilmiştir. Yaşları 1 ile 46 arasında olan multipl travmalı 21 felaketzedeye girişim yapılmıştır ve ikisine kardiopulmoner resüsitasyon uygulamak gerekmiştir. İğne bir olguda radial kemiğin distal ucuna, diğerlerinde ise tibial tuberositas bölgesine yerleştirilmiştir Tüm olgularda iğne yerleştirme başarısı %100'dür. Bu seride girişimden kaynaklanan ağrı veya komplikasyon gözlenmemiştir. Sonuç olarak, darbeli ve yüksek hızlı otomatik bir alet olan kemik enjeksiyon tabancası kullanımı kitle felaketzedelerinde intraosseöz yol üzerinden sistemik dolaşıma ulaşma konusunda başarılı bulunmuştur. Ayrıca, bilinci yarı açık yaralılarda keskin ve küçük trokar iğnesinin darbeli penetrasyonu kemiğin ağrı hissini minimuma indirmektedir.
