Every boy with acute onset of scrotal pain and swelling, requires an immediate evaluation. When duration of the pain is brief and history and physical examination suggest thai torsion is the most likely diagnosis, urgent surgical exploration without additional imaging studies is recommended. When it is not possible to definitely diagnose or exclude the diagnosis of testicular torsion, or when the duration of pain is greater than 12 hours, then diagnostic imaging can provide significant information. Color Doppler sonography is preferable to nuclear imaging for the evaluation of children with acute serotinus. When normal or increased blood flow is present, scrotal exploration is not required. In decreased blood flow, surgery is recommended.

Ani başlayan skrotal ağrı ve şişlik yakınmaları olan her erkek çocuğun acil olarak değerlendirilmesi gereklidir. Ağrı yeni başlamışsa, anamnez ve fizik muayene testis torsiyonu lehineyse herhangi bir göriintüleme yöntemine gerek kalmadan acil cerrahi eksplorasyon önerilmektedir. Testis torsiyonu ekarte edilemiyorsa, veya ağrının süresi 12 saatten uzunsa görüntüleme yöntemleri tanıda oldukça yararlıdır. Renkli Doppler ultrasonogra.fi noninvazif olması, çabuk yapılması, anatomik yapı hakkında bilgi vermesi ve günümüzde her merkezde bulunması nedeniyle sintigrafiye üstünlük sağlamaktadır. Etkilenen testisin kan akımı karşı testise göre normal veya artmışsa cerrahi girişime gerek yoktur. Kan akımı azalmışsa veya kan akımı hakkında bilgi elde edilememişse cerrahi girişim gereklidir.
