The development of a postoperative lymphocele after renal transplantation is a well-described complication that occurs with relative frequency. Management options have previously included simple aspiration, percutaneous imaging-guided drainage with catheter placement, and operative marsupialization of the cyst into the peritoneal cavity. Because these collections are often multiloculated, catheter drainage may be of limited value, and the recurrence rate is unacceptably high. The operative approach is the most definitive method and is still considered the treatment of choice. Laparoscopic treatment of pelvic lymphocele secondary to kidney transplant has gained popularity in the last few years. We now report a case of posttransplanted lymphocele that was successfully drained into the peritoneal cavity using laparoscopy. We believe this is an effective alternative route to provide internal drainage for lymphoceles.

Renal transplantasyon sonrası gelişen, iyi bilinen bir komplikasyon da lenfoseldir. Tedavide kullanılan yöntemler iğne aspirasyonu, perkutan drenaj, skleroterapi ve internal drenajdır. Perkutan drenaj, lenfoselin genellikle mültioküle olması nedeniyle başarısız, kalmakta ve olay sıkça tekrarlamaktadır. Tedavide cerrahi yaklaşım en iyi metoddur. Endoskopik yöntemle lenfoselin peritona fenestrasyonu son yıllarda popülarite kazanmış bir yöntemdir. Burada, ünitemizde laparoskopik yöntemle internal drenaj yapılan komplike lenfoselli bir renal transplant olgusu sunulmuştur. Lenfosel tedavisinde laparoskopik fenestrasyon yöntemini, açık cerrahi girişime efektif bir alternatif olarak kullanılabileceğine inanmaktayız.
