Background : Brucellosis is a zoonotic disease transmitted infected animals to humans. The kidney involvement is rare seen in brucellosis. In this study we aimed to investigate the renal involvement in brucellosis. With clinical and laboratuary data Materials an d methods : We included the patients diagnosed brucellosis who admitted to the Van State Hospital Department of Infectious Disease and Yüzüncü yd University Training Hospital Department of Internal Medicine. Brucellosis was diagnosed by standard tube agglutination titers equal or higher than 1/160 with compatible clinical findings. Complete urine analysis, urine culture and biochemical analysis were performed. Results : The patients were between the ages 14¬65 years (mean: 33.4 ± 14.1) and half of them were male.Increase in urine urobilinogen was found in 18 patients (56.3 %), proteinuria in 16 (50 %-degrees of proteinuria were moderate in 10 patients, mild in 6 patients), bilirubiuria in 15 patients (46.9 %), microscopic hematuria in 9 patients (28.2 %) and ketonuria in 7patients (21.9 %). In addition serum sodium levels weer detected decreased in 5 cases (15.6 %), and increase in one case (3.1 %). We did not find prominent renal failure and asit-base metabolism disorders in any patient. Conclusion : Brucellosis can cause more renal involvement move than expected rates. We determined prominent proteinuria (mild or moderate), bilirubinuria, and an increase in urine urobilinogen. We rarely found deterioration in sodium metabolism.

Amaç : Bruselloz, enfekte hayvanlardan insanlara bulaşabilen zoonotik bir hastalıktır. Seyrek olarak böbrek tutulumu yapabilmektedir. Bu çalışmada bruselloz tanısı konulan olgularda klinik ve laboratuvar verilen yardımı ile böbrek tutulumunu incelemeyi amaçladık. Yöntem ve gereçler : Bu çalışmaya Van Devlet Hastanesi Enfeksiyon Hastalıkları Polikliniğine ve Yüzüncü Yıl Üniversitesi İç Hastalıkları Polikliniğine başvuran, Wright testi 1/320 'nin üzerinde pozitif olan veya belirgin bruselloz kliniği ile birlikte Wright testi 1/160'in üzerinde pozitif olan 32 hasta dahil edildi. Tüm hastaların tam idrar tetkiki ve idrar kültürü, biyokimyasal incelemeleri yapıldı. Bulgular : Çalışmaya yaşları 14-65 (33.4 ± 14.1) arasında değişen 16'sı erkek, 16'sı bayan 32 hasta dahil edildi. Olgularımızın 18'inde (% 56.3) idrarda ürobilinojen artışı, 16'sında (% 50) proteinüri (10'unda (%31.3) orta derecede, 6'sında (% 18.8) hafif derecede), 15'inde (%46.9) bilirubinüri, 9'unda (% 28.2) mikroskopik hematüri, 7'sinde (% 21.9) ketonüri saptandı. İlaveten beş (% 15.6) olguda hiponatremi, bir (% 3.1) olguda da hipernatremi bulundu. Hiçbir olguda belirgin böbrek yetmezliğive asit-baz denge bozukluğuna rastlanmadı. Sonuç : Bruselloz, bilinenden daha fazla oranlarda böbrek tutulumu yapabilen sistemik bir enfeksiyon hastalığıdır. Brusellozlu olgularda dikkat çeken oranlarda hafif ve orta derecede proteinüri, bilirubinüri ve idrarda ürobilinojen artışı saptandı. Seyrek olarak da hiponatremi ve hipernatremiye rastlandı.
