We aim to evaluate the nutritional status and the main causes of malnutrition in uremic children. Study group consisted of 29 children (15 male, 14 female) with end stage renal disease (ESRD). There were ten (5 male, 5 female) predialysis (preD), ten (5 male, 5 female) peritoneal dialysis (PD) and nine (5 male, 4 female) hemodialysis (HD) patients with a mean age of 12.6+4.6 years. All the patients had lower weight and height standart deviation score (SDS) than normal Turkish children. When the patients were evaluated from the aspects of relative weight (RW), normal nutritional state was detected in most of the patients (25/29), moderate malnutrition (RW%74) in one, and obesity (RW>%110) in 3 patients. PreD patients had mild malnutrition (%90±3,7), PD patients had moderate malnutrition (%89,5±6,4) and HD patients had severe malnutrition (%82,9±5,5) according to the height for age standarts. The results of our study show that the mean height, weight SDS and the height for age value are the most usefull parameters for the evaluation of growth and nutritional status in children with ESRD. Low energie intake, and impaired energie-protein expenditure are the major risk factors influence growth and development in our patients with ESRD.

Çalışmamızda kronik böbrek yetersizliği (KBY) olan çocuk hastalarda beslenme durumunun değerlendirilmesi ve malnutrisyona yol açan başlıca nedenlerin belirlenmesi amaçlandı. Çalışmaya yaş ortalaması 12.6±4.6 yıl olan 29 çocuk (15 erkek, 14 kız) alındı. Henüz diyalize girmeyen 10 çocuk prediyaliz (PreD) grubunu, periton diyalizi yapılan 10 çocuk PD grubunu ve hemodiyaliz uygulanan 9 çocuk HD grubunu oluşturdu. Tüm çocukların tartı ve boy standart deviasyon skoru (SDS) değerleri sağlıklı Türk çocuklarına göre düşük bulundu. Relatif tartı değerine göre hastaların çoğu (25/29) normal beslenme durumu gösterirken, yaşa göre boy değerlendirildiğinde PreD hastalan hafif malnutrisyon (%90.0±3.7), PD hastalan orta malnutrisyon ((%89.5±6.4), daha düşük enerji-protein tüketimi gösteren HD hastaları ağır malnutrisyon (%82.9±5.5) durumu göstermekteydiler. KBYtandı çocuk hastalarda tartı ve boy-SDS ve yaşa göre boy değerlerinin büyüme ve gelişmenin değerlendirilmesinde en güvenli parametreler ve düşük enerji tüketimi ve bozulmuş protein dengesinin malnutrisyona yolaçan başlıca faktörler olduğu sonucuna varıldı.
