A Case of Chronic Renal Failure Due to Posterior Urethral Valve

We present a 15-year-old boy admitted with complaints of polydipsia, pollakiuria, enuresis nocturna and retardation in growth and development. He was diagnosed with chronic renal failure, with a glomerular filtration rate of 22 ml/min/1.73 m2. Christmas tree bladder was shown in voiding cystourethrogram, urodynamic evaluation revealed spastic bladder and posterior urethral valve was observed by cystoscopy. Obstruction due to posterior uretral valve and recurrent urinary tract infections were the presumed causes of the chronic renal failure. Autoaugmentation cystoplasty + Mitrofanoff procedure were per formed by pediatric urologist. The aim of these procedures were to save the renal reserve, to delay the progression to end-stage renal failure, to correct the incontinence and to prepare the bladder for a future kidney transplantation. After a short period of follow up, glomerular filtration rate increased, recurrence frequency of urinary tract infections decreased and incontinence improved. We want to draw attention to a cause of chronic renal failure which can be treated by early diagnosis and also the role of autoaugmentation cystoplasty + Mitrofanoff procedure in slowing down the progression to end-stage chronic renal failure.

On beş yaşındaki erkek vaka yaklaşık on yıldır çok su içme, sık idrar yapma ve geceleri altını ıslatma ve büyüme gelişme geriliği şikâyetleri ile başvurdu. Fizik muayenede ağırlık ve boy üçüncü persantilin altında İdi. Vaka, yapılan tetkikler sonucunda, (GFR:22 ml/dk/1.73 m2) kronik böbrek yetmezliği (KBY) tanısı aldı. Voiding sistoüretrografisinde çam ağacı görünümünde mesane, ürodinamîsinde spastik mesane, sis-toskopisinde posterior üretral valf (PUV) görüldü. KBY, PUV'a bağlı obstrüksiyona ve geçirilmiş olası enfeksiyonlara bağlandı. Vakada kalan böbrek rezervini korumak, son donem böbrek yetmezliği gelişmesini geciktirmek, spastik mesaneyi düzelterek İnkontinansı ortadan kaldırmak ve ileride olası böbrek transplantasyonuna mesaneyi hazırlamak için pediyatrik üroloji tarafından otoogmentasyon sistoplastisi + Mitrofanoff kanalı (appendix kullanılarak) uygulandı. Açılan stomadan altı saatte bir temiz aralıklı kateterizasyon yapılmaktadır. Hastanın kısa süreli izleminde böbrek fonksiyon bozukluğunda düzelme olduğu (GFR:32.2ml/dk/1.73m2), İnkonti-nansının düzeldiği, idrar yolu enfeksiyonlarının azaldığı görüldü. Bu vaka sunumu ile, önlenebilir nedenlerle gelişen KBY'ye dikkat çekmek ve ayrıca otoogmentasyon sistoplastisi + Mitrofanoff kanalının böbrek yetmezliğinin ilerlemesini durdurmada etkili olduğu gösterilmek istendi.
