Non-steroid antiinflamatuar (NSAİ) ilaçların böbrek üzerinde ciddi yan etkileri vardır. Akut böbrek yetmezliği bu yan etkilerin önemli bir bölümünü oluşturmaktadır. NSAİ ilaçların böbrek fonksiyonları üzerine olumsuz etkilerini açıklayan bazı mekanizmalar bilinmesine rağmen, en çok kabul gören mekanizma siklooksijenaz (COX) yolunun son ürünlerinden olan PGE2 ve PGI2 gibi vazodilatatör pros-taglandinlerin bu ilaçlarla bloke edilmesidir. Mo-leküler çalışmalar siklooksijenazın iki izoformu olduğunu göstermiştir. Bazal durumlarda böbrekten ve diğer organlardan eksprese edilen COX1 ve inflamasyon durumlarında eksprese edilen COX2. COX2'nin makula densa ve medulladan salındığı da gösterilmiştir. COX2'nin inhibisyonu renal fonksiyonlarda akut azalmayı ortaya çıkar¬maktadır (1).
