Pathophysiology of Ischemic and Toxic Tubular Necrosis

Akut böbrek yetmezliği (ABY) böbrek fonksiyonlarında kısa süre içinde izlenen bozulma ile ka-rakterize bir tablo olup, günümüzde de mortalite oranlarında geçmiş yıllara kıyasla belirgin bir düzelme sağlanamamıştır. Bu nedenle patofizyolojisinin anlaşılması ve patogenezine yönelik tedavilerin geliştirilmesi önem kazanmaktadır. İskemik ve toksik sebepler ABY'nin en sık se¬bepleri olmaya devam etmektedir ve bu rakamlar is-kemik akut tübüler nekroz (ATN) için %50, toksik ATN için de %35 olarak rapor edilmektedir (1). Hastanede gelişen ABY'ler arasında ATN %45 ve prerenal ABY %21 oranında yer tutmaktadır (2). Özellikle hastanede yatan hastalarda izlenen iskemik ABY, çoklu organ yetmezliği ve sepsis tablosu ile birlikte bulunur.
