EFFECT OF UNIVERSITY OF WISCONSIN SOLUTION ON ISCHEMIA-REPERFUSION DAMAGE IN RENAL PRESERVATION

The preservation solution in cadaveric renal trans¬plantation has been shown to be one of the major fac¬tors influencing the incidence of postoperative allog-raft dysfunction. The purpose of our study was to exa¬mine the ultrastructural and biochemical adverse ef¬fects of cold ischemia in the renal preservation with simple cold storage, and to investigate the efficacy of University ofWiscosin (UW) solution in the preventi¬on of these adverse effects. Rabbits in the first group underwent nephrectomy and perfused with the Euro-Collins (EC) solution, af¬ter 48 and 96 hours reperfusion was done with the blood of the same animal. Perfusion and preservation in the second group was done with the UW solution. The samples from the kidney were examined under electron-microscopy (EM) and lipid peroxidation and glutation levels were analysed. In the EM examination of the first group; red blood cell (RBC) trapping, eryt-hrocyt agglutation and endothelium adhesion were the major findings after 24 h duration of preservati¬on. Endothelial defects and erythrocyte extravasation were observed in the kidneys underwent reperfusion. Although RBC trappingwas seen in the second group, agglutination and adhesionwas not seen in the 24 h preservation time. Mitochondria of the tubulus and microvilli were better protected and the erythrocyte extravasation was less prominent. We concluded that UW solution causes less dama¬ge to the mitochondria and microvilli by maintaining hight glutation levels; prevents the adhesion of eryt-hrocytes to the endothelium membrane during 24 h preservation; thereby lessening the damage caused by ischemic reperfusion.

Kadaverik renal transplantasyonda postoperatif allograft disfonksiyonu insidansını etkileyen en önemli faktörlerden biri olarakprezervasyon amacıyla kul-lanılansolüsyon gösterilmektedir. Çalışmamızın amacı 96 saatlik basit soğuk koruma ile renalprezervas-yonda soğuk iskeminin allograft üzerine olumsuz etki¬lerini ultrastrüktürel ve biokimyasal olarak öğrenmek ve bu hasarların önlenmesinde Wisconsin Üniversite¬si (UW) solüsyonununetkinliğini araştırmaktadır. Birinci grupta tavşanlara nefrektomi, Euro-Collins (EC) solüsyonu ile perfüzyon, 48 ve 96 saat sonra ay¬nı hayvandan alınmış kanla perfüzyon uygulandı, ikinci grupta perfüzyon ve prezervasyon UW solasyo-nu ile yapıldı. Böbreklerden alınan parçalarda elekt-ron-mikroskopik inceleme yapılarak lipit peroksidas-yon ve glutatyon düzeylerine bakıldı. Birinci grubun EM incelemesinde eritrosi birikimi, 24 saatprezer-vasyon sonrası eritrositlerin aglütinasyonu ve endote-liyuma yapışması esas bulgu idi. Reperfüzyon yapılan böbreklerde ise endotel zedelenmesi ve eritrosit eks-travazasyonuna rastlandı. İkinci grupta eritrosit biri¬kimine rastlanmasına rağmen, 24 saat prezervasyon boyunca agliltinasyon veadhezyon görülmedi, tubulus mitokondriumlan ve mikrovUlusları daha iyi korun¬muştu, reperfüzyon sırasında eritrosit ekstravazasyo-nu seyrekti. UW solüsyonunun renal dokuda glutatyon düzeyini yüksek tutarak, mitokondriumların vemikrovillusların daha az hasar görmesine neden olduğu, 24 saat pre-zervasyon boyunca eritrositlerin endotel membranına adhezyonunu önlediği ve dolayısıyla iskemik reper-füzyon hasarlarını hafiflettiği görüşüne vardık.
