EARLY INDICATORS OF RENAL DAMAGE IN ESSENTIAL HYPERTENSION

Essential hypertension (HT) leads to certain functional and structural chances in kidney that possible to detect at early stage. To investigate these effects, our study performed on 209 HT patients with m an age 48.2±9 years. Their systolic and diastolic blood pressure were 159.9^14 and 100.4±8 mmHg. 114 healthy subjects with mean age 46.6±9years were included as controls. Serum uric acid (UA), urinary protein, albumin (UAE), N-acetyl-Q-D-glucosaminidase (NAG), p-2 microglobulin $2M were measured as renal damage indicators. In HT group UA, UAE, NAG (P<0.000), §2M, proteinuria (P<0.005) were higher versus to control. Also,%59 retinopathy (HR) and %28 overt microalbuniuria (MA) were observed. UA was higher in male(P<0.000), serum creatinin (P<0.000), urinary NAG/Cr e atinin, $2M/Creatinin ratio and proteinuria (P<0.05) were higher in female.

Yetişkin populasyonda sık rastlanan ve renal yetmezlik nedenleri arasında önemli yeri olan esansiyel hipertansiyonun (HT) erken evrede böbrek üzerine etkileri belirlenebilmektedir. Bu amaçla renal fonksiyonu normal, yaş ortalaması 48.2±9 yıl, sistolik ve diyastolik kan basıncı 159.9±14 ve 100.4±8 mmHg olan 209 HTlu olgu ve yaş ortalaması 46.6±9 yıl olan 114 sağlıklı birey çalışmaya alındı. Böbrek etkilenme göstergelerinden serum ürik asit (ÜA), idrarda mikroprotein, üriner albumin (ÜAE), N-acetyl-$-D-glucosaminidase (NAG), $-2mikroglobulin ($2M) atılımı araştırıldı, %76'sı hafif- orta düzeyde olan, %59 hipertansif retinopati(HR) ve%28 'nde mikroalbuminüri (MA) saptanan hastalarda ÜA, ÜAE, NAG, (P<0.000) $2M ve proteinüri (P<0.005) kontrol grubundan yüksek bulundu. Erkeklerde ÜA yüksekliğine (P< 0.000) karşı kadın hastalarda serum kreatinin (P<0.000), idrarda NAG/kreatinin ve $2M/kreatinin oranları, proteinüri daha yüksek (P<0.05) bulundu.
