THE FACTORS THAT EFFECT RECURRENCES, SURVIVAL AND SATISFACTION OF PATIENTS WITH EARLY STAGE BREAST CARCINOMA TREATED WITH BREAST CONSERVING THERAPY

Objective: To demonstrate the demographic properties, local control and survival and contentment of therapy by patients with early breast cancer treated with breast conserving therapy. Methods: Between 1999 and 2005, twenty six patients' records were retrieved. Results: The average age of 26 patients was 51 (+/-8) years, of which 14 were premenopausal. The average follow up time was 30 (+/-21) months. Invasive ductal carcinoma was reported in 25 patients and while one patient had Tx, 17 had T1 and 8 had T2; 17 patients had N0, 4 N1, 4 N2; 2 had nuclear grade I, 15 II, 8 III. Eight patients had less than 1cm of clear margin. Fourteen of the patients had estrogen, whereas 13 had progesterone, and 7 had c erb b2 receptors. Seventeen patients received chemothera¬py, 21 had hormonotherapy. One patient died because of multi¬ple metastases. The mortality rate was 3.8% and no local recur¬rences were observed. Only 18.18% of patients were unsatisfied with therapy and 9.09% were unhappy with the new shape of their breast. Conclusion: Since the number of patients are few, we could not clarify the criteria that effect local and systemic recurrence and mortality. However, there was a positive correlation between the education level and satisfaction both with therapy (p=0.04) and the new shape of the breast (p=0.024).

Amaç: Meme koruyucu ameliyat yapılan erken evre meme kanserli hastaların demografik özelliklerini, lokal kontrolü ve yaşam süresini etkileyen faktörleri ve tedaviden memnuniyetlerini belirlemek. Yöntem: 1999 ile 2005 yılları arasında ameliyat edilen 26 hasta retrospektif olarak incelendi. Bulgular: Yaş ortalaması 51 (+/-8). Hastaların 14'ü premenopozal dönemdeydi. Ortalama takip süresi 30 (+/-21) aydır. Yirmi beş hastanın patoloji rapor sonucu invazif duktal karsinom; biri Tx, 17'si T1 ve 8'i T2'dir. Aksilla incelemelerini 17'si N0, 4'ü N1, 4'ü N2; nükleer gradları 2' sinin I, 15'inin II ve 8' inin III' tür. Bir hastanın bilgilerine ulaşılamamıştır. Sekiz hastada cerrahi sınır 1 cm'-den azdı. On yedi hastanın 14'ünde östrojen, 13'ünde progest-ron, 10 hastanın 7'sinde ise c erb b2 pozitifdi. Hastaların 17'si ke-moterapi, 21'i hormonoterapi aldı. Hastalarımızda ölüm oranı %3.80, lokal nüks 0 ve uzak metastaz %3.80' tür. Hastaların %18,18'i tedaviden, %9.09'u memenin şeklinden memnun olmadığını belirtti. Sonuç: Az sayıdaki hasta grubumuzda lokal ve sistemik rekürensi ve mortaliteyi etkileyen faktör tespit edilemedi. Eğitim durumları ile yapılan tedaviden (p=0.04) ve yeni meme şeklinden memnuniyet (p=0.024) arasında pozitif ilişki bulundu.
