While the history of hand infections reaches over to the Hippocrates era, Giuseppe Giannini in the beginning of 18th cen¬tury, determined that microscopic particles were responsible for infections. Hand infections tend to spread and fall into anatomic compartments (division). Infections affect compartments by direct trauma, spreading from adjacent compartment or hematogenous way. Infection of closed spaces in the hand tend to cause more rapid tissue necrosis and abscess formation than superficial infec¬tions. Staphylococcus aereus is responsible for 50-80% of pyo-genic hand infections and cellulitis while viruses, fungal and mycobacterial infections and infection like manifestations are uncommon so far. The treatment of hand infections was success¬ful after the invention of antibiotics. According to Kanavel, the important step for treatment of hand infections is the early and accurate diagnosis. Surgical drainage should be added to treat¬ment when the infection advances to supurative phase.

El enfeksiyonlarının tarihi Hipokrat dönemine uzanır. 18. yüzyılın başlarında Giuseppe Giannini enfeksiyonların yaşayan mikrosko-pik partiküller tarafından meydana getirildiğini gösterdi. El enfeksiyonları anatomik kompartmanlara yerleşme ve yayılma eğilimindedir. Enfeksiyon, kompartmanlara doğrudan yaralanma, komşu kompartmandan ya da uzak bir odaktan hematojen yayılımla ulaşır. Elin enfekte kapalı boşlukları, abseleşme ve nekroz yüzeyel enfeksiyonlardan daha hızlı gelişir. Eldeki piyojenik enfeksiyonlar ve selülitin %50-80'inden Staphylococcus aureus sorumludur; virüs, mantar ve mikobakteri enfeksiyonları ile enfeksiyon benzeri tablo¬lar ise daha nadir görülür. Antibiyotiklerin bulunmasıyla el enfeksiyonlarının tedavisinde başarı elde edilmiştir. Kanavel'e göre, el enfeksiyonlarının tedavisinde erken ve doğru tanı önemli bir basa¬maktır. Enfeksiyon süpüratif faza ilerlediğinde cerrahi drenaja baş¬vurulmalıdır.
