State and trait anxiety among family physicians: a case control study

Objective: The aim of this study is to evaluate anxiety levels of family physicians and also to compare these with different special¬ties of medicine. Methods: Data were collected via a questionnaire including demographical characteristics and the State-Trait Anxiety Inventory of Spielberger et al. which determines anxiety in a spe¬cific situation and as a general trait in two twenty-item scales. Student's t test, one-way ANOVA and chi-square tests were used for statistical analysis. Results: The mean age of the 178 doctors were 34.19±8.25 years. More than half of them (52.2%, n=93) were female and 62.4% (n=111) were family physicians. The doctors' mean levels of state and trait anxiety were 37.26±7.95, and 41.27±7.25, respectively. State anxiety levels were similar according to age, gender, institution, and specialty (p>0.05). Within the family physicians although state anxiety wasn't different in regard to age, gender, and institution (p>0.05), trait anxiety was significant¬ly higher in family physicians working at universities, young ones and trainees (p<0.05). The ratio of the family physicians that "never and slightly" wanted to be again a family physician again with a second chance was 54%. Job promotion and higher income in family medicine were not expected by 62.1% and 78.3%, respectively. Conclusions: State and trait anxiety average scores in family prac¬tice and the other specialties were similar. It can be concluded that generally high levels of trait anxiety of family physicians and their answers about career items may be the result of a future concern which is important for the academic development of this specialty.

Amaç: Bu çalışmanın amacı aile hekimlerinin anksiyete düzeyini belirlemek ve diğer uzmanlık alanları ile karşılaştırmaktır. Yöntem: Veriler Spielberg ve arkadaşlarının geliştirdiği, duruma özel ve genel anksiyete düzeyini belirleyen, yirmişer maddelik Durumluk Kaygı Ölçeği, Sürekli Kaygı Ölçeği ve sosyo-demografik özellikleri içeren bir soru formu ile toplandı. istatistiksel analizlerde Student's t-testi, one-way ANOVA testi ve ki-kare testi kullanıl¬dı. Bulgular: Çalışmaya katılan 178 hekimin yaş ortalaması 34,19±8,25 yıldı. Yarıdan çoğu (%52.2, n=93) kadın, %62.4'ü aile hekimi (n=111) idi. Tüm hekimlerin durumluk ve sürekli anksiyete düzeyleri sırasıyla 37.26±7.95 ve 41.27±7.25 idi. Aile hekimlerinin yaş, cinsiyet, çalışılan kuruma göre değerlendirilen durumluk anksiyete düzeyi diğer dal hekimlerinden farklı değildi (p>0.05). Aile hekimlerinin durumluk anksiyete düzeyi yaş, cinsiyet ve kuruma göre değişmezken (p>0.05), sürekli anksiyete düzeyi gençlerde, üniversitede çalışanlarda ve asistanlarda anlamlı derecede yüksekti (p<0.05). "Yeniden karar verme olanağım olsa bu kariyeri seçmezdim" (hiç ve biraz diyenlerin) oranı %54'tü. Aile hekimleri grubu¬nun %62.1'inin kariyerinde yükselme, %78.3'ünün ise para ka¬zanma beklentisi yoktu. Sonuç: Aile hekimliği ve diğer dalların durumluk ve sürekli kaygı düzeyi ortalamaları birbirine benziyordu. Aile hekimlerinin sürek¬li kaygı düzeyinin yüksek olması ve kariyer ile ilgili yanıtları gele¬cek kaygısı taşıdıklarını düşündürebilir ki bu da dalın akademik gelişimi açısından önemli bir konudur.
