42 year old woman with G7P3L3C3 reported that she got pregnant although she had a DMPA injection 3 months ago and requested a legal abortion in our clinic. She claimed that she had her first DMPA injection 3 months ago in a family planning unit in another education and research hospital. She did not have any worries about not having her periods then on because she was told that this method is safe, highly effec¬tive and frequently causes amenorrhoea. When she applied to the family planning unit with subsequent complaints of nau¬sea and bloating, she was reassured that such symptoms were common in the first few months of injections. When she realized that her abdomen enlarged, she took a pregnancy self test kit which revealed positive. Then she applied to our Family Planning Unit in Haydarpaşa Numune Education and Research Hospital, with an unwanted pregnancy and a request of legal abortion. Her physical examination unveiled a 17 weeks pregnancy. When she was questioned, it was understood that she was not called for a check visit after her DMPA injection following her legal abortion 12 weeks ago. Conclusion: We concluded that all cases whom legal abor¬tions are performed must be reevaluated for the risk of unsuc¬cessful evacuations or complications. Although it seems as a method failure in the first place, it is obvious that the preg¬nancy was not terminated.

Olgu: 1961 doğumlu, G7P3Y3C3 olan Ş.K., 3 aylık korunma iğnesi kullandığı halde gebe kaldığını ve aldırmak istediğini söyleyerek Haydarpaşa Numune Hastanesi Aile Planlaması Üni-tesi'ne başvurdu. ilk kez 3 ay önce başka bir eğitim hastanesinin Aile Planlaması Merkezinde DMPA uygulanan olguya bu yöntemin çok güvenilir ve etkili bir koruma sağladığı, ancak adetten kesebileceği söylenmiş; bu nedenle adet görmediği için endişelenmemişti, bir kez de bulantı ve şişkinlik nedeniyle aynı merkeze başvurduğunda bunun ilk aylarda normal olduğu söylenmişti. Karnının büyümesi üzerine şüphelenerek gebelik testi yaptıran ve pozitif çıkması nedeniyle bize başvuran ol¬gumuz, gebeliğini sonlandırmak istediğini söyledi. Muayene ve USG ile 17 haftalık gebe olduğunu saptadığımız olgunun öyküsü derinleştirdiğimizde, kendisine 12 hafta önce yasal tahliye sonrası aynı gün DMPA yapıldığı ve kürtaj sonrası kontrole çağrılmadığı tespit edildi. Sonuç: Yasal tahliye yapılan tüm olguların gebeliğin devamı ya da komplikasyonların saptanabilmesi açısından kontrole çağı¬rılması gerektiği sonucuna vardık. ilk başta DMPA'ya bağlı yöntem başarısızlığı gibi görünen, ancak gebeliğin sonlandırı-lamaması nedeniyle gebeliğin devamı olan bu olgunun sunul¬masının uygun olduğunu düşünüyoruz.
