Leptomeningeal Carsinomatosis Arising From Gastric Cancer: A Case Report

The infiltration of the leptomeninges by malignant cells is a common complication of cancer and is called leptomeningeal carcinomatosis. The most common associated malignancies are breast and lung cancer, melanoma, lymphomas and leukaemias. Primary gastrointestinal tumours has been rarely reported with leptomeningeal carcinomatosis. A 59-year-old man was admitted to our hospital with the symptoms of fatique and weight loss. The endoscopy showed a vegetan mass of the less curvature of stomach. The biopsy revealed a poor-differentiated adenocarcinoma. However, the patient complained of a headache and balance disorder afterwards. The cytology examination of the cerebrospinal fluid supported the diagnosis of signet ring cell carcinoma.

Leptomeningeal karsinomatozis malign hücrelerin leptomeninksleri infiltrasyonuyla gelişen, tüm kanserlerde görülebilecek yaygın bir komplikasyonudur. Sıklıkla meme ve akciğer kanseri, melanoma, lenfoma ve lösemilerle ilişkilidir. Primer gastrointestinal sistem tümörlerinden gelişen az sayıda olgu bildirilmiştir. Olgumuz 59 yaşında erkek hasta halsizlik, kilo kaybı yakınmaları ile hastaneye başvurdu. Uygulanan endoskopi sonrasında mide küçük kurvatür hizasında vejetan büyüme göstermiş tümöral kitle saptandı. Alınan biyopsi materyallerinin incelenmesi sonucunda az diferansiye adenokarsinoma tanısı konuldu. Hastada daha sonra başağrısı ve dengesizlik yakınmaları gelişti. Gönderilen BOS materyalinin sitolojik incelemesinde taşlı yüzük hücreli karsinoma tanısı verildi
