Clinical Comparison of Icare Tonometer and Goldmann Applanation Tonometer

The aim of this study is to compare the intraocular pressure (IOP) readings taken with the Icare tonometer with the Goldmann applanation
tonometer in normal patients. This study involved 64 eyes of 32 patients who was referred to the Ophthalmology clinic of the Uludag University Medical Faculty, for refraction and have no other ophthalmological problem except a refractive error. Two observers obtained IOP
readings with Icare tonometer before a local anestetic drop and then also with Goldmann applanation tonometer (GAT). These two readings
are compared with each other. For this purpose the analysis of percentage change, intraclass correlation, analysis of Pearson correlation and
the Mann- Whitney tests are used; the avarege, median, standart deviation and minimum-maximum values are calculated and the level of
significance is accepted as 0.05. In addition, central corneal thickness values of all the patients are obtained with Pentacam®. Also for the
assesment of these measurements the Mann Whitney test and analysis of Pearson correlation are used and teh level of significance is also
accepted as 0.05. The differance between the two methods (ICare-GAT) determined as for right eyes 0 (ranges -0.40 and 0.82) and for left
eyes 0 (ranges between-0.29 and 1.00). There was no significant difference statistically between the IOP measurements of two methods (p>
0.05) The intraclass correlation coefficiency was significantly high with ICare and GAT methods (p<0.01, r=0.769).There was no significant
correlation with pachymetry (Pentacam®) measurements between both of the methods. In our study, there was no significant differance
between the two methods of IOP measurements. Icare tonometry easy to use, the measurement is simple and fast, does not require topical
anesthesia, because according to Goldman applanation tonometry has emerged that this may serve more comfortable for the patient and the
physician.

Bu çalışmanın amacı; göziçi basıncını ölçmek için geliştirilen Icare tonometresi ve Goldmann aplanasyon tonometresinin normal gözlerde
karşılaştırılmasıdır. Bu çalışma, UÜTF göz polikliniğine refraksiyon muayenesi amaçlı başvuran ve refraktif kusur dışında göz problemi
olmayan toplam 32 hastanın 64 gözünü içerdi. Hastaların önce topikal anestezi kullanılmadan Icare tonometresi ile daha sonra Goldmann
aplanasyon tonometri yapılarak göz tansiyonları ölçüldü. Her iki ölçüm kendi aralarında karşılaştırıldı. Bu amaçla yüzde değişim analizi,
intraklas korelasyon, Pearson korelasyon analizi ve Mann-Whitney testleri ile ortalama, medyan, standart sapma değerleri ve minimummaksimum değerler hesaplandı, anlamlılık düzeyi 0,05 olarak kabul edildi. Ayrıca bütün hastalara Pentacam® cihazı ölçüm yapılarak santral
kornea kalınlığı değerleri elde edildi. Bu ölçümlerde Mann Whitney testi ve Pearson korelasyon analizi ile değerlendirildi, anlamlılık düzeyi
yine 0,05 olarak kabul edildi. İki metod arasındaki (Icare-Goldmann) göziçi basınç farkı sağ gözde 0 (-0,40 ile 0,82 arasında), sol gözde 0 (-
0,29 ile 1.00 arasında) olarak saptandı. Yüzde değişimleri bakımından İcare ve GAT ölçümleri arasında istatistiksel olarak anlamlı fark
bulunamadı (p> 0.05) Yine Icare ile GAT arasında kuvvetli korelasyon (p<0.01, r=0,769) görüldü. Çalışmamızda; her iki teknikten elde
edilen göziçi basınç değerleri arasında anlamlı bir fark saptanmadı. Icare tonometri kullanımı kolay, ölçümü basit ve hızlı olması, topikal
anestezi gerektirmemesi nedeniyle Goldman aplanasyon tonometriye göre hasta ve hekim açısından daha konforlu olabileceği görüşü ortaya
çıkmıştır.
