This review summarises evidence relevant to the involvement of N-nitroso compounds in the induction of tobacco-related cancers in man. Cancers of the oral cavity, lung, oesophagus and pancreas will be discussed. Human exposure to nitrosamine compounds in tobacco products is more intense and widespread than from any other known source. Many substances, such as N-nitroso com/munds. 4-<N-nitrosomethylamino)-l-(3 pyridyl)-1-butanone and N'-nitrosonornicotine and polynuclear aromatic hydrocarbons available in tobacco may involve in causing cancers of the oral cavity, lung, oesophagus and pancreas m tobacco users. NNK and its metabolites have been shown to induce tumours in experimental animals. Both NNK and benzo[a/pyrene active the K-ras oncogenes with mutations in codon 12 were detected m 13 of 45 adenocarcinomas obtained from the lungs of smokers. The reduction or elimination of these substances from tobacco products would probably lead to a decrease m the incidence of these cancers. /Journal of Turgut Özal Afedical Center l(4):342-345,]994/

Hu çalışma, sigaranın kanserojen etkileri ile ilgili bilimsel yayınların incelenmesi ve bunun ışığında sigara yapımında kullanılan tütün kalitesi ile ilgili uygulanabilir bazı önerilerin ortaya konması amacı ile yapılmıştır. Yapılan epıdemiyolojik çalışmalar sigara içiminin ağız, gırtlak, akciğer, ösofagus ve idrar kesesi kanserlerine sebep olabileceğini göstermektedir. Sigarada bulunan polisiklik hidrokarbonlar, N-nitrosaminler ve heterosiklik aminler muhtemel kanser yapıcı maddeler olup, deney hayvanları ve insan doku kültürü çalışmaları bunu doğrular niteliktedir. Özellikle nitrosaminler bakımından zengin olan siyah tütünün bu riski daha da artırabileceği ifade edilmiş olup polisiklik aromatik hidrokarbonlar ve N-nitroso gibi tütün orijinli olan maddelerin fare derisinde kanserine sebep olabileceği gösterilmiştir. Bu maddelerin ayrıca K-ras onkojenlerını aktif/eştirerek kodon 12'de nokta mutasyonlarma sebep olduğu bulunmuştur. Sigaranın kanser yapıcı elkisısinın azaltılabilmesi için, tütün tiplerini nitrat içeriklerine göre sınıflandırarak tütünlerde bulunan nitrat bileşiklerini düşürücü tedbirler alınması toplum sağlığı yönünden önemli görülmektedir.[Turgut Özal Tıp Merkezi Dergisi l(4):342-345.1994]
