Abstract (2. Language):
Failure t o thrive (FTT) is widely used t o describe inadequate growth
in early childhood, but no consensus exists concerning the definition
of FTT. FTT is not a disorder, it is a physical sign that a child is receiv¬
ing inadequate nutrition f o r optimal growth and development. It
may result f r om a variety of " o r g a n i c " and "nonorganic" causes or
a combination of b o t h . More than 8 0 % o f children w i t h FTT do not
have an underlying medical disorder. No routine laboratory tests are
ordered in t he initial work-up of failure t o thrive unless suggested by
the history or physical examination. Infants and children w h o develop
FTT are at increased risk o f long-term g r o w t h , development, and
behavior problems. Children diagnosed w i t h failure to thrive need
t o receive 150 percent of the recommended daily caloric intake f or
their expected weight for age. A team approach and long-term
close follow-up are necessary for t he successful management of
these children.

Erken çocukluk dönemindeki yetersiz büyümeyi belirtmek için kullanılan
serpilme yetersizliğinin tanımı ile ilgili görüş birliği yoktur.
Serpilme yetersizliği bir hastalık değil, çocuğun optimal büyümesi
ve gelişmesi için yetersiz beslendiğini gösteren bir bulgudur. Bir
grup organik bozukluktan ya da organik olmayan nedenlerden ve¬
ya her ikisinin birlikteliğinden kaynaklanabilir. Bu çocukların
% 8 0 ' i n d e n fazlasında organik bir neden saptanamaz. Başlangıçta
yapılacak rutin bir laboratuar test listesi yoktur, öykü ve fizik mu¬
ayeneye göre tetkikler planlanmalıdır. Serpilme yetersizliği olan süt
çocukları ve daha büyük çocuklar uzun dönemde büyüme, gelişme
ve davranış problemleri açısından risk altındadırlar. Serpilme yetersizliği
olan çocuk yaşa göre vücut ağırlığı esas alınarak hesaplanan
günlük kalori ihtiyacının % 1 5 0 ' s i n e gereksinim duyar. Bu çocukların
başarılı bir şekilde tedavisi için ekip yaklaşımı ve uzun süreli ya¬
kından izlem greklidir
