The Effect of Diurnal Blood Pressure Pattern on Heart Rate Recovery After Exercise Testing in Treated Hypertensives

It was investigated that whether or not hypertensive patients with controlled blood pressure (BP) and blunted nighttime BP fall show attenuated heart rate (HR) recovery after exercise. All subjects underwent a maximal exercise testing according to Bruce protocol. The heart rate
recovery (HRR) was defined as the difference in HR between the end of the exercise and 1 minute after exercise. 24-h ambulatory BP monitoring determined the circadian BP pattern. Patients with dipper BP status had lower average nighttime systolic-diastolic BP values; and
higher values of percentage of systolic-diastolic BP reduction compared to non-dippers. Dippers had also higher HRR values than nondippers (34.36±14.55/min-32.48±8.60/min, p=0.025). There were no correlation between HRR and average nighttime systolic-diastolic BP
and percentage of systolic-diastolic BP reduction. To sum up there is statistically significant but not clinically so important association
between non-dipper phenomenon and a slowed HRR. This may demonstrate the importance of BP control rather than correcting the nondipper condition.

Kan basıncı (KB) kontrollü olan ve gece ölçülen kan basıncında yeterli düşme olmayan (non-dipper) hipertansiflerin egzersiz testi sonrası
kalp hızı toparlanmasında gecikme gösterip göstermediği incelendi. Tüm hastalara Bruce protokolüne göre maksimal efor testi uygulandı.
Kalp hızı toparlanması (KHT), efor testi bittiğindeki kalp hızı ile bir dakika sonraki kalp hızı arasındaki fark olarak tanımlandı. Sirkadiyen
KB, 24 saatlik ambultuvar KB monitörizasyonu ile tespit edildi. Dipper KB profili olan hastaların “non-dipper” olanlara göre gece ortalama
sistolik ve diyastolik KB daha düşük, sistolik ve diyastolik KB gece gündüz farkı yüzdesi ise daha yüksekti. Dipper olanların KHT değerleri
“non-dipper” bireylerden daha yüksekti (34.36±14.55/dak-32.48±8.60/dak, p=0.025). Ortalama gece sistolik-diyastolik KB değerleri ve KB
gece gündüz farkı yüzdesi ile KHT arasında korelasyon yoktu. Sonuç olarak “Non-dipper” durum ile KHT gecikmesi arasında istatistiksel
olarak anlamlı ancak klinik açıdan çok da önemli olmayan bir ilişki vardır. Bu bulgular “non-dipper” durumun düzeltilmesinden çok KB
kontrolünün önemini gösterebilir.
