Efficiency of New Generation Shock Wave Lithotripsy Device for the Treatment of Lower Ureteral Stones

Aim: The most appropriate approach of lower-end urinary stones of the ureter is not clear. The therapies vary according to the size of stones and consist of medical treatment (drugs), Extracorporeal Shock VVave Lithotripsy (ESVVL), Uretero-Renoscopy (URS) and occasionally open ureterolithotomy. İn this study, vve aimed to investigate the efficiency of a new generation ESVVL devices and its side effects for the therapy of lower-end urinary stones vvith collected retrospective data of theconcerned patients.
Materials and methods: Betvveen January 2007 and January 2011, 86 patients enrolled to this study are admitted to Urology Department and diagnosed vvith distal ureteral stones. Fifty-six of the patients vvere male (% 65,1) and 30 of them vvere female (% 34,9), children vvere not enrolled into the study. The mean age vvas 41 (21-63) and the mean diameter of stones vvas 7,6 mm (5-12 mm). Nevv generation electro-magnetic Storz Medical Modulith SLK device is used for vvhole procedure. The method vvas mainly ultrasonographic focusing in order to ensure convenience, efficiency, and patient safety.
Results: VVİthin one month follow-up, 80 of 86 distal ureteral stone patients (% 93.03) vvere stone free. For the remaining 6 patients; due to unbreakable stones and remnants, URS vvas applied and stones vvere fully removed. No analgesic required for 71 patients (% 82.6). Neither general, spinal, epidural anesthesia, nor narcotic drugs vvere applied to any patient.
Conclusion: Our study shovved that it is possible to obtain high success and lovver rate of complications vvith nevv generation electro-magnetic ESVVL devices for the treatment of lovver-end urinary stones.

Amaç: Üreter alt uç taşlarının tedavisinde, en uygun tedavi yaklaşımı henüz tam olarak netlik kazanmamıştır. Uygulanan tedavi yöntemleri taşın büyüklüğüne göre, medikal tedavi, Extracorporeal Shock VVave Lithotripsi (ESVVL) ve üreteroskopi (URS) ile taşın ekstraksiyonu ve çok nadiren başvurulan üreterolitotomidir. Çalışmamızda üreter alt uç taşlarına ESVVL uygulanan hastaların verilerini retrospektif olarak inceleyerek üreter alt uç taşı tedavisinde kullanılan yeni kuşak ESVVL1 nin başarısını ve yan etkilerini araştırmayı amaçladık.
Gereç ve Yöntem: Ocak 2007 ile Ocak 2011 tarihleri arası üroloji polikliniğine baş vuran ve üreter alt uç tanısı alan 86 hasta çalışmaya dahil edildi. Seksen altı hastanın 56'sı (% 65.1) erkek, 30' u (% 34.9) kadındı, çalışmaya çocuk hastalar dahil edilmemiştir. Hastaların yaşları 21 ile 63 arasında (ortalama 41 yıl) idi. Taşların çapı 5-12 mm (ortalama 7.6 mm) arasında değişmekte idi. ESVVL işleminde yeni jenerasyon elektromanyetik Storz Medical Modulith SLK kullanıldı. Hem ultrasonik hem de X-Ray floroskopik odaklama sistemi içeren bu cihazda kolaylık, etkinlik ve güvenlik nedeniyle ağırlıkla ultrasonik odaklama uygulandı.
Bulgular: Bir aylık takibi yapılabilen 86 distal üreter taşlı hastanın 80 (% 93,03)' ünde taştan arınma saptandı. Altı hastanın taşının kırılmaması veya rezidü saptanması nedeniyle üreterorenoskopi (URS) ile taşlar temizlendi. Hastaların 71 (% 82,6)' sında herhangi bir analjezi ihtiyacı olmadı. Hiçbir hastaya genel, spinal ve epidural anestezi, sedasyon ve narkotik analjezi uygulanmadı.
Sonuç: Yeni jenerasyon ESVVL ile üreter alt uç taşlarının tedavisinde daha yüksek oranda başarı ve daha düşük oranda komplikasyonlarile karşılaşıldığı ortaya konulmuştur.
